Most often work hours are 11-7pm or 12-8pm\n<h2>Responsibilities</h2>\n<ul>\n <li>Conduct comprehensive, home-based behavioral health assessment inclusive of the CANS (Child and Adolescent Needs and Strengths standardized assessment tool).</li>\n <li>Develop and coordinate treatment planning.</li>\n <li>Provide intensive family therapy.</li>\n <li>Identify community resources and develop natural supports for youth and parent/guardian/caregiver</li>\n <li>Provide complete care coordination:</li>\n <li>Attend necessary meetings, to include, IEP meetings, hospital discharge meetings, Care Planning Team (CPT) meetings for members enrolled in ICC. </li>\n <li>Provide collateral contacts for case coordination (phone and face to face)</li>\n <li>Collaborate with care coordinator (for youth enrolled in ICC services) and/or behavioral service providers for treatment planning</li>\n <li>Provide referrals and linkage to appropriate supports and services</li>\n <li>Coordinate and communicate to collaterals all aftercare planning </li>\n <li>Provide coaching in support of decision making in both crisis and non-crisis situations.</li>\n <li>Provide skills training for youth and parent/caregiver/guardianProvide phone crisis response during working hours and participate in on-call coverage rotation.</li>\n <li>Provide member/service recipient transportation as needed in connection with service delivery.</li>\n <li>Conduct member outreach as needed to engage members and families. </li>\n <li>Participate in Risk management/safety planning.</li>\n <li>Complete all required documentation in a timely manner.</li>\n <li>Attends and actively participates in supervision and staff meetings.</li>\n <li>Develop and maintain working relationships with CSA (Community Service Agencies as part of Mass Health’s managed care contractors) and community partners.</li>\n</ul>\n<p> </p>\n<h2>Qualifications</h2>\n<ul>\n <li>Master’s level degree in social work, counseling or related field required </li>\n <li>Experience working with children/families and adolescents in child development, childhood mental illness, or family issues preferred.</li>\n <li>Must complete in Home Therapy training program upon acceptance of position and prior to providing services.</li>\n <li>Certification to administer CANS-MA version.</li>\n <li>Must be proficient in use of standardized documentation procedures.</li>\n <li>Must be able to perform each essential duty satisfactorily.</li>\n <li>Ability to communicate effectively verbally and in writing.</li>\n <li>Ability to develop and maintain satisfactory working relationships with employees and providers.</li>\n <li>High energy level, superior interpersonal skills and ability to function in a team atmosphere.</li>\n <li>Commitment to Advocates values and mission.</li>\n <li>Must hold a valid drivers’ license.  </li>\n <li>Must have access to an operational and insured vehicle and be willing to use it to transport members.</li>\n</ul>\n<p><em>Advocates is committed to cultivating a diverse and welcoming community where everyone feels respected and valued.
