This role ensures timely clinical response to urgent patient needs overnight, on weekends, and on holidays, helping to maintain continuity and quality of care throughout the entire hospice experience.</span></p>\n<p><br></p>\n<p><span>This position is ideal for an experienced hospice nurse who is passionate about patient comfort and can confidently provide care independently during on-call hours.</span></p>\n<p><br></p>\n<p><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\">PRIMARY RESPONSIBILITIES</span></span><br></p>\n<ul>\n<li><span>Respond to patient calls, triage needs, and deliver compassionate, timely hospice care.</span></li>\n<li><span>Assess and manage symptoms including pain, shortness of breath, agitation, and more.</span></li>\n<li><span>Provide skilled nursing care in accordance with the plan of care and hospice protocols.</span></li>\n<li><span>Administer medications and perform treatments as prescribed by the physician.</span></li>\n<li><span>Communicate and collaborate with the interdisciplinary hospice team to ensure quality, coordinated care.</span></li>\n<li><span>Support families and caregivers through education, emotional support, and guidance.</span></li>\n<li><span>Accurately document all interactions, assessments, interventions, and changes in condition.</span></li>\n<li><span>Travel to patient residences (e.g. Fresno Guest Homes or other in-home locations) to deliver in-person care when necessary.</span></li>\n<li><span>Participate in hospice team meetings and ongoing staff development.</span></li>\n</ul>\n<p><br></p>\n<p><span><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\">QUALIFICATIONS</span></span> </span><br></p>\n<ul>\n<li><span>Active Registered Nurse (RN) license in the State of California.</span></li>\n<li><span>Minimum of one (1) year experience in hospice or palliative care preferred.</span></li>\n<li><span>Certification in Hospice and Palliative Nursing (CHPN) is </span><span>preferred, but not required</span><span>.</span></li>\n<li><span>Strong clinical assessment skills, including ability to make autonomous decisions in the field.</span></li>\n<li><span>Demonstrated empathy and communication skills with patients and families in emotional or end-of-life situations.</span></li>\n<li><span>Reliable transportation, valid driver’s license, and proof of auto insurance (travel is required).</span></li>\n<li><span>Comfortable with electronic documentation and use of EMR systems.</span></li>\n<li><span>Bilingual in Spanish, Hmong, or other languages spoken by our patient population are a plus.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\">Work Schedule</span></span></p>\n<ul>\n<li><span>Clocked In </span><span>5pm-9pm:  In Person in facility visits</span></li>\n<li><span>Unpaid</span><span> 9pm-9:30pm Meal Period </span></li>\n<li><span>Clocked In </span><span>9:30pm-1:30am: Office or home Paperwork / Documentation</span></li>\n<li><span>(Clock In when called) </span><span>1:30am-6:00am: On Call Response as needed</span></li>\n<li><span>Guaranteed 8hrs/day + any calls between 1:30am-6am</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\">Work Environment &amp; What We Offer:</span></span></p>\n<ul>\n<li><span>Work is conducted  at both a typical office environment and on-site at assigned facilities, engaging with residents, families, and staff.</span></li>\n<li><span>Competitive wages and a comprehensive benefits package including Health Insurance (Medical, Dental, and Vision), Paid Sick Leave, and a 401k &amp; Deferred Profit-Sharing Plan</span></li>\n<li><span>Opportunities for ongoing professional development and career growth.</span></li>\n<li><span>A dynamic, supportive, and collaborative work environment.</span></li>\n</ul>",
