This position involves providing structured services and skill-building activities, conducting outreach, and connecting clients to community resources. The Day Hab Navigator will work closely with clients to develop personalized plans that address their needs and goals.</span></span><span> </span></p>\n<p><br></p>\n<p><span><span>Key Responsibilities:</span></span><span> </span></p>\n<ul>\n<li><span><span>Client Support:</span></span><span><br><span>Assist</span><span> clients in developing and implementing day habilitation plans that reflect their interests, capabilities, and needs. Provide prompts, reminders, guidance, and role modeling in areas such as socialization, personal hygiene, exercise, nutrition, and behavior.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Skill Building:</span></span><span><span> Develop and provide structured services and activities designed to meet the needs of clients, enhancing their skills and level of independence.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Community Integration:</span></span><span><span> Encourage clients to </span><span>participate</span><span> in community activities and events, fostering a sense of belonging and integration.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Outreach:</span></span><span><span> Conduct outreach in the community to engage clients and connect them to resources. Meet clients in various settings, including shelters, parks, and community events.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Advocacy:</span></span><span><span> Advocate on behalf of clients to ensure they receive the necessary support and services. </span><span>Assist</span><span> clients with applications for public benefits and other resources.</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Documentation:</span></span><span><br><span>Maintain </span><span>accurate</span><span> records of client interactions, progress, and services provided.
