This position is ideal for someone who is person-centered, adaptable, community-focused, and motivated to help others achieve greater independence and quality of life. As a Site Coordinator, you will support individuals in identifying what is important to them while ensuring their health, safety, and personal goals remain a priority.</p><p>This position is responsible for carrying out Individual Support Plans (ISPs) for assigned individuals and providing day-to-day guidance, mentorship, and leadership to Direct Support Professionals (DSPs) and Assistant Site Coordinators (ASCs). Site Coordinators are expected to model Positive Practices, promote a supportive team environment, and help ensure quality services are delivered consistently.</p><p>Availability is required for day, evening, overnight, and weekend hours.
