color: rgb(0, 0, 0);\"><strong>The RN Hospice House/IPU Duties are:</strong></span></p><p style='margin:0in;font-size:16px;font-family:\"Aptos\",sans-serif;'><span style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">&nbsp;</span></p><ul style=\"list-style-type: disc;\"><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.&nbsp;</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Assess and evaluates the patient and family including the physical, spiritual, and psychosocial needs.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Develops and implements an individualized plan of care including creating measurable patient-centered goals in collaboration with the patient and family, that consistently is updated and reviewed to reflect assessment findings to ensure quality patient care. &nbsp; &nbsp;&nbsp;</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Reviews and updates plan of care including interventions, medication profile, visit frequency to meet the patient&rsquo;s changing needs.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Assess and reports to physician changes of patient condition, level of care needs, and&nbsp;facilitate&nbsp;determining&nbsp;eligibility for services.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Collaborates and participates as member of hospice team in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Team (IDT) meetings, to ensure coordination of services.&nbsp;</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Documents physicians&rsquo; orders when received in electronic health&nbsp;record&nbsp;to provide&nbsp;accurate&nbsp;information and ensure safe delivery of services.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Supervises,&nbsp;teaches&nbsp;and evaluates care provided by Personal Care Specialists,&nbsp;Volunteers&nbsp;and other care givers to ensure compliance to the plan of care.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Documents assessments, interventions and all changes thoroughly at the point of service in the electronic health record and synchronizes this documentation for each patient according to Ohio&rsquo;s Hospice standards to ensure quality communication with patient&rsquo;s Interdisciplinary Team.&nbsp;</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Communicates handoffs such as SBAR assessments and other methods, timely to ensure communication is relayed appropriately for continuation of quality care.&nbsp;</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Assists&nbsp;in other activities and departments when requested to ensure complete and thorough patient care for all Ohio&rsquo;s Hospice patients.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Participates in Continuous Quality Improvement activities to support the quality of Ohio&rsquo;s Hospice services.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Precepts&nbsp;new staff&nbsp;to support professional relationships with newly hired team members as&nbsp;required.</li></ul><p style='margin:0in;font-size:16px;font-family:\"Aptos\",sans-serif;margin-left:.5in;'><span style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\"><strong>&nbsp;</strong></span></p><ul style=\"list-style-type: disc;\"><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Graduate of an accredited RN college; Bachelor of Science in Nursing&nbsp;(BSN) preferred</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Active Registered Nurse license in the State of Ohio without any board actions.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Two years acute care nursing experience preferred&nbsp;</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Certification in Hospice &amp; Palliative Medicine (CHPN) preferred and will be made available to Ohio&rsquo;s Hospice employees who wish to obtain certification.</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">CPR Certified</li><li style=\"font-family: arial, sans-serif; color: rgb(0, 0, 0);\">Computer skills sufficient to properly document services and care.</li><li style=\"font-family: arial, sans-serif;
