Coordinate behavioral health care within Brevard Health Alliance (BHA). Deliver evidence-based mental health services, collaborate with the multidisciplinary team in the FQHC, participate in care coordination, and support integrated primary care goals.</p><p><strong><u>GENERAL EDUCATION REQUIREMENTS</u></strong></p><p>Master&rsquo;s degree in Social Work, Mental Health Counseling, Psychology, or a related behavioral health field from an accredited program.&nbsp;</p><p><strong><u>ADDITIONAL QUALIFICATIONS</u></strong></p><ul style=\"list-style-type: disc;margin-left: 0in;\"><li>Current, active Florida license as a Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC).&nbsp;</li><li>Passing score on the appropriate national clinical examination (e.g., ASWB clinical for LCSW or equivalent for LMHC).</li><li>Experience providing individual, family, and group psychotherapy in a community health or primary care setting; FQHC experience preferred.&nbsp;</li><li>Knowledge of integrated behavioral health practices and collaborative care models.&nbsp;</li><li>Proficient with Electronic Medical Records (EMR).&nbsp;</li><li>Excellent verbal and written communication and interdisciplinary collaboration skills.&nbsp;</li><li>Strong cultural competence and experience working with diverse, underserved populations.&nbsp;</li><li>Automated office and PC competency required.&nbsp;</li><li>Current Basic Life Support (BLS)/CPR certification from the American Heart Association or American Red Cross.</li></ul><h4 style=\"margin-left:0in;\"><u>PRIMARY ACCOUNTABILITIES</u></h4><ul style=\"list-style-type: disc;margin-left: 0in;\"><li>Provide direct clinical services: assessment, diagnosis, and evidence-based individual, family, and group psychotherapy appropriate to patient needs and scope of practice.&nbsp;</li><li>Develop, document, and update treatment plans and progress notes in the EMR in a timely manner according to BHA policy.&nbsp;</li><li>Participate in warm handoffs, case consultations, and care planning with primary care providers, behavioral health team members, and other multidisciplinary staff to ensure integrated care.&nbsp;</li><li>Conduct mental health screenings and brief interventions in primary care visits as needed.&nbsp;</li><li>Make and coordinate appropriate referrals to community resources and specialty services; follow-up to support continuity of care.&nbsp;</li><li>Facilitate group therapy and psychoeducational groups as assigned.&nbsp;</li><li>Provide patient and family education regarding mental health diagnoses, treatment options, coping strategies, and community resources.&nbsp;</li><li>Participate in quality improvement activities, program development, chart reviews, and clinical meetings to support clinical outcomes and operational goals.&nbsp;</li><li>Maintain professional licensure, participate in continuing education, and adhere to ethical and regulatory standards.&nbsp;</li></ul><ul type=\"disc\"><li>Perform other related duties as assigned&nbsp;</li></ul><ul style=\"list-style-type: disc;margin-left: 0in;\"><li>Takes ownership and represents Brevard Health Alliance&rsquo;s mission and organizational goals established by the executive committee.
