We are building a Program of All-Inclusive Care for the Elderly (PACE) to bring life-changing, culturally competent care to low-income seniors—supporting them as they age safely and with dignity, right in their communities. Based on the PACE model, SpringLight Health will offer coordinated medical care, transportation, meals, social activities, medication management, and caregiver support—all tailored to each individual’s needs. Job Summary: The Center Director provides day-to-day operational and clinical leadership for the PACE Center, ensuring a safe, welcoming, and high-performing environment for participants, caregivers, and staff. This role leads key PACE Interdisciplinary Team (IDT) functions and partners closely with the Executive Director and Medical Director to deliver high-quality, person-centered care. Note: 1.0 FTE equals 40 hours per week. Essential Job Functions Lead the daily operation of the PACE Center to ensure a safe, clean, functional, and welcoming environment for participants, staff, and visitors. Serve as a key member of the PACE leadership team; partner closely with Executive Director and Medical Director on program performance and readiness. Coordinate and facilitate core IDT processes and meetings, including care planning and follow-up tracking. Oversee and coordinate participant needs across the PACE Center, home environment, and contracted/community services to ensure seamless care delivery. Ensure participant rights and responsibilities are upheld; support effective grievance and appeals processes in partnership with appropriate leaders. Support 24-hour care coordination processes for participants (per PACE requirements), including escalation pathways and handoffs. Provide direct clinical leadership as an RN, including triage support and the ability to flex into urgent operational/clinical gaps in a fast-paced environment. Oversee clinic and ADHC/day center operations in close coordination with the Medical Director and clinical leaders. Supervise center staff (interdisciplinary care team (RN, PT, OT, Nutrition, social work), day center staff, recreation, home care coordination, transportation coordination, dietary, and other center support roles), including hiring, training, coaching, performance management, and scheduling. Manage supplies, equipment, and vendor relationships supporting the center’s operations; coordinate physical plant, facilities maintenance, and safety. Support emergency preparedness planning, drills, and ongoing monitoring in alignment with federal/state requirements. Ensure compliant use of the Electronic Health Record (EHR) and adherence to HIPAA and relevant PACE regulations. Participate in audit preparation and response (CMS/DHCS and other oversight), and support implementation of corrective actions. Promote continuous quality improvement and participant satisfaction through measurement, feedback loops, and rapid operational improvements. Represent the PACE program professionally in the community; maintain effective relationships with partners and stakeholders. Minimum Qualifications Current, unrestricted Registered Nurse (RN) license (California). Demonstrated ability to triage and operate comfortably in a fast-paced clinical/operational environment. Minimum 3 years of progressive leadership/supervisory experience in a healthcare setting. Experience working with frail older adults (or willingness to complete required training prior to independent practice). Experience coordinating across multidisciplinary teams (e.g., nursing, social work, rehab, recreation, dietetics, home care, transportation). Strong meeting facilitation, documentation follow-through, and operational execution skills. Comfort with EHR workflows and technology-enabled care coordination. Preferred Qualifications Prior PACE experience, or related experience in ADHC/CBAS, geriatrics, managed care, home-based care, or post-acute settings. Prior experience as a Director of Nursing, post-acute facility director, nurse manager, or comparable leadership role. Critical care experience and emergency department (ED) nursing background. Bachelor’s degree in nursing or health-related field; Master’s degree preferred. BLS certification. Bilingual ability aligned with the community served.
