    "description": "<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">The<span> </span><span style=\"font-weight: bold\">Patient Support Nutritionist</span><span> </span>provides comprehensive Patient care in the community setting that assists patients to achieve optimum health and quality of life in situations of chronic disease.  The Patient Support Nutritionist is responsible for the provision and management of effective Patient care to individuals and/or families in the community or in-Patient Support Programs. The Patient Support Nutritionist works in adherence to the Policies and Procedures set by Axios International and works in accordance with established standards of Patient practices according to local and international rules and regulations. </span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\"> </span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt; font-weight: bold\">Service Responsibilities</span></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Based on the agreed-upon services and workflow, the Patient Support Nutritionist provides skilled Patient services in accordance with the orders of a physician and under a plan of care. Duties include but not limited to:</span></p>\n<ul>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Telephonic follow up consultation</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Ensures clients’ menu selections are in compliance with prescribed diet order, nutritional adequacy, age, cultural background, and religious practice.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Writes and modifies menus as needed according to client diet prescription using established resources/guidelines.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Updates, monitors, and follows-up on all pertinent client information like diet order changes, food allergies, food preferences, and requests.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Records all assigned data such as client meal census and floor stock requests on paper logs or spreadsheet.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Resolves client requests and complaints or relays to appropriate staff.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Directs members and staff of Food and Nutrition Services office.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Serves as a dietary resource for nursing and other staff.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Provides general information on diet utilizing approved materials and resources.</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Education and counselling, consistent with the type/nature of treatment and/or care management need.</span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Providing general Patient care</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Supervision/health teaching and support for family and caregivers on regular basis</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Reporting Adverse Events and follow the country rules and regulation regarding AE reporting procedure, and reporting any concerns /changes in patient health to the program and/or treating physician Familiarity with Patient equipment and ability to provide Patient assessments to patients</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt; font-weight: bold\">Organizational Responsibilities</span></p>\n<ul>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Reflecting and interpreting the Axios Vision, Mission and Core Values in his/her own work with enthusiasm and commitment</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Acting in accordance with relevant legislation and company Policies, Standards and Procedures</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Developing and maintaining respectful, cooperative working relationships to contribute to the integrated, seamless delivery of services to patients, families and to our clients</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Understanding his/her role and responsibility in maintaining a safe workplace and reducing workplace injuries</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Effectively assisting the Program Managers in managing the Patient Support Program(s) by liaising with hospitals, distributors, pharmacies, and patients</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Working closely with the Axios UAE staff to implement the required service(s) for the enrolled patients in the program(s)</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Carrying out home visits for patient care as required</span></li>\n<li><span style=\"font-family: arial, helvetica, sans-serif; font-size: 10pt\">Preparing timely monitoring reports (monthly) with patient numbers and logistics as well as quality assurance reports (quarterly) for internal review.
