    "description": "<h2>Our Company</h2>\n<p>Amerita</p>\n<p></p>\n<h2>Overview</h2>\n<p>The Pharmacist performs pharmaceutical care activities for customers/patients within the service area; monitors, documents and reports patient outcomes; provides or coordinates the dispensing of medications, home medical equipment and ancillary supplies within the service area; and participates in on-call schedule.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals</li>\n <li>Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice</li>\n <li>Adheres to all appropriate standards of accreditation and participates in quality assurance activities</li>\n <li>Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution</li>\n <li>Reviews, receives and verifies all new patient orders and documents on the respective order form</li>\n <li>Assists the physician and other involved healthcare professionals with: Planning and implementing patient care in the home</li>\n <li>Setting therapeutic objectives for drug therapy</li>\n <li>Establishing parameters for monitoring the safety and efficacy of drug therapy</li>\n <li>Preparing regular progress reports</li>\n <li>Conducts and documents initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs.</li>\n <li>Monitors the outcomes of drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.</li>\n <li>Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.</li>\n <li>Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.</li>\n <li>Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.</li>\n <li>Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences.</li>\n <li>Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules</li>\n <li>Supervisory Responsibility: No</li>\n</ul>\n<p> </p>\n<h2>Qualifications</h2>\n<p>EDUCATION/EXPERIENCE• Bachelor of Science in Pharmacy• Doctor of Pharmacy preferred• Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy• Experience with compounding infused drugs</p>\n<p>LICENSE/CERTIFICATION/OTHER SPECIAL REQUIREMENTS• Registered Pharmacist License in state(s) of practice• Willing to obtain and maintain compliance with additional state licensure outside of home state, to support company operation</p>\n<p> </p>\n<p>KNOWLEDGE/SKILLS/ABILITIES• Good organizational and time-management skills• Participation and membership in professional societies and organizations is desirable• Excellent verbal and written communication skills• Schedule flexibility</p>\n<p> </p>\n<p>TRAVEL REQUIREMENTSPercentage of Travel: 0-25%</p>\n<p> </p>\n<p>**To perform this role will require frequently sitting, standing, walking and typing on a keyboard with fingers, and occasionally bending, reaching, and climbing (stairs/ladders).
