    "ExternalDescriptionStr": "<p><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: 11pt; line-height: 115%;\"><strong><u>ABOUT THE JOB</u></strong></span></span></p><p><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: 11pt; line-height: 115%;\">The role of the hospice social worker is to deliver varied social work services to Hospice patients and their families. This may include emotional, spiritual, and psychosocial assessments, as well as ongoing counseling, bereavement services and community education, outreach, and referral. The hospice social worker is an integral part of the hospice interdisciplinary team.&nbsp;This is a casual position with no set hours per week.</span></span></p><ul style=\"list-style-type: disc;\"><li style=\"line-height: normal; margin-bottom: 0in;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: 11pt;\">Functions as a member of the Hospice Interdisciplinary Team</span></span></li><li><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: 11pt;\">Performs initial psychosocial assessment on every hospice patient to identify needs, determine the medium of service delivery, and complete follow through to ensure provision for social, emotional, and psychological needs of the patient and family.</span></span></li><li><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: 11pt;\">Provides psychosocial input into the development of the patient plan of care.</span></span></li><li><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: 11pt;\">Assist the team in understanding the significance of psychosocial factors on the overall health and well-being of the family unit facing serious illness.</span></span></li><li><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: 11pt;\">Assists with the emergency or direct placements to SNF/NF/AFL facilities as needed.</span></span></li><li><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: 11pt;\">Provides orientation and ongoing education to staff and volunteers on social work principles.</span></span></li><li style=\"line-height: normal; margin-bottom: 0in;\"><span style=\"font-family: Arial, sans-serif;\"><span style=\"font-size: 11pt;\">Completes necessary forms and documentation regarding assessment and observations for inclusion into patient's medical record.</span></span></li><li style=\"line-height: normal;
