*This position comes with a $10,000 Sign on Bonus for qualified candidates!
*Territory will be Naples to Marco Island and will reimburse mileage at the current federal rate of 72.5 cents per mile.
Job Summary:
The licensed Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.
Job Duties:
Provide PT services prescribed by a physician, which can be safely provided in the home to include:
a. Initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points (G333, G336).
b. Developing and revising the plan of care in consultation with the physician and other care-team members (G162, G186).
c. Providing therapeutic treatments and evaluation of equipment needs as applicable.
d. Advising and consulting with the family and other agency personnel as applicable regarding medical equipment, HEP, safety Issues (G143, G188).
e. Preparing clinical and progress notes (G187).
f. Evaluating outcomes of care.
g. Supervising Therapy Assistants and Home Health Aides as appropriate (G190).
h. Planning for discharge.
Will follow all policies and procedures including safeguarding the privacy of protected patient health information and the Compliance Program.
Participating in in-services and QA programs as requested. (G189).
All other duties as assigned.

Qualifications
Education/Experience:
Graduate of an accredited PT program
1 year experience in Physical Therapy minimum
Certificates, Licenses, Registrations:
Valid Florida Driver’s License.
Licensed Physical Therapist in the state of Florida
Valid CPR certification
Supervisory Responsibilities: None.
Computer Skills:
Knowledge of basic typing skills, data entry and word processing software, database software and general knowledge of e-mail.
Language Skills:
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write speeches and articles for publication that conform to prescribed style and format. Ability to effectively present information to top management, public groups, and/or boards of directors.
Mathematical Skills:
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Reasoning Ability:
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
