Make a lasting impact.</em></p>\n<p> </p>\n<h3><strong>Position Overview:</strong></h3>\n<p>The Director of Social Services oversees admissions, discharge planning, and social service programming to support resident well-being and regulatory compliance.</p>\n<p>In this role, you will:</p>\n<ul>\n <li>Lead the admissions and discharge planning process</li>\n <li>Support residents and families through transitions of care</li>\n <li>Supervise and guide social services staff</li>\n <li>Collaborate with interdisciplinary teams on care planning</li>\n <li>Ensure compliance with all regulatory and documentation requirements</li>\n <li>Act as a liaison between residents, families, and external agencies</li>\n</ul>\n<p> </p>\n<p><strong>Elderwood Benefits:</strong></p>\n<ul>\n <li><p>Competitive pay aligned with your experience</p></li>\n <li><p>Tuition Reimbursement Programs</p></li>\n <li><p>Shift Differentials</p></li>\n <li><p>Full Suite of Health Benefits – Medical, Dental, Vision</p></li>\n <li><p>PTO & Holiday Package</p></li>\n <li><p>401K with Employer Matching</p></li>\n <li><p>Employee Referral Bonus</p></li>\n <li><p>Opportunity to make a meaningful impact on residents’ daily lives</p></li>\n <li><p>Supportive, team-oriented work environment</p></li>\n <li><p>Employee appreciation initiatives throughout the year</p></li>\n <li>On-the-job training and growth opportunities</li>\n</ul>\n<h2>Responsibilities</h2>\n<ul>\n <li>Assumes responsibility for admission process for new residents as described in Section B of the Social Services Department Manual.</li>\n <li>Serves as the Unit Social Worker for designated residents as described in Section B of the Social Services Department Manual, or delegates this responsibility according to current staffing pattern.</li>\n <li>Serves as the Discharge Planner of the facility as described in Section B of the Social Services Manual; completes SCREEN upon discharge of resident.</li>\n <li>Coordinates PASARR Level II Annual Review of residents.</li>\n <li>Implements and oversees the policies and procedures of the Social Services Department in the areas of admission, social work, transfer and discharge services.</li>\n <li>Participates in development of a comprehensive, inter-disciplinary health care and discharge plan for residents; attends Resident Care Planning and Resident/ Family Care Plan Review meetings and informs other disciplines of residents or family members who plan to attend.</li>\n <li>Provides support and assistance to the Unit Social Worker in work with residents and families.</li>\n <li>Supervises and evaluates the performances of the social services staff.</li>\n <li>Works with the Social Work Consultant in matters of social services intervention or consultation.</li>\n <li>Ensures all necessary documentation required for the Social Services Department.</li>\n <li>Interprets to the professional staff of the facility the purpose and objectives of the Social Services Department.</li>\n <li>Explains the Bill of Rights for Residents and Resident Abuse Reporting at General Orientation meetings, as scheduled.</li>\n <li>Coordinates facility cooperation with the Ombudsman.</li>\n</ul>\n<p> </p>\n<h2>From</h2>\n<h2>Up to</h2>\n<h2>Qualifications</h2>\n<ul>\n <li>Minimum of a bachelor’s degree in social work from an accredited School of Social Work required.</li>\n <li>One year of supervised social work experience in a health care setting working directly with individuals and one year of supervisory capacity preferred.</li>\n <li>Experience in casework required.</li>\n <li>Prior work with elders in a community or long-term care setting is preferred.</li>\n <li>Must have skills in communication, assessment and social work methods and techniques.</li>\n <li>Have a working understanding of social systems along with the ability to implement pertinent state and federal regulations.</li>\n <li>This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
