    "description": "<p><strong>Comunilife Inc. </strong>is looking for experienced an <strong>Case Manager</strong> for its congregate program in the <strong>Bronx, Prospect Residence. </strong>The Case Manager&rsquo;s main responsibility is to provide supportive services to assigned program participants in a housing setting. The type of support includes assisting with defining personal, vocational, and educational goals and using a person-centered / Harm Reduction model that supports individuals in their recovery, hopes, and opportunities.&nbsp;<em><strong>This position is eligible for Comunilife&#39;s generous benefits and paid time off package including medical, dental, vision, life insurance, retirement employer contributions after 2 years, tuition reimbursement and professional development opportunities.</strong></em></p>\r\n\r\n<p><strong>Schedule:</strong></p>\r\n\r\n<p><strong>Sun.-Thurs. 9am - 5pm</strong></p>\r\n\r\n<p><strong>Essential Duties:</strong></p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<ul>\r\n\t<li dir=\"ltr\">\r\n\t<p>Ensure that each client is receiving appropriate and effective case management services.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Conduct face to face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit&nbsp; and one contact per week.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Provide assistance in accessing primary medical care; assistance with adherence to medication regiments; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living.&nbsp;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Conduct reassessments and review service plans as needed on a quarterly basis to ensure that services are appropriate for the clients&rsquo; current needs.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Ensures that each client has psychosocial.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Provide crisis intervention to clients and their families as needed.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Maintain ongoing awareness of clients&rsquo; medical conditions so that services are appropriate to changing needs.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Provide counseling and orientation regarding tenant&rsquo;s rights and responsibilities to maintain and retain housing.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and provide transportation when necessary.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>In collaboration with vocational counselors and entitlement specialists provide advocacy &amp; referrals to educational, training and/or employment programs and/or job placement in the private sector.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Assist clients in locating alternative housing options, when a client&rsquo;s health so requires.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Provide and coordinate all supportive services as needed.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Collaborate with all providers within and outside of Comunilife involved with the client/family.&nbsp;</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Maintain and complete required documentation including all contacts made with the client and any contact on their behalf.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Participate in weekly supervision.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Submit a weekly schedule of field visits.</p>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<p>Participate in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors.</p>\r\n\t</li>\r\n</ul>\r\n",
