Title : Program Manager - Breast and Cervical Cancer Prevention

Direct Reports : Medical Case Manager(s), Health Promotion Program Coordinator(s), Patient Navigator(s), and/or Intern(s)
Summary of Position:
The Program Manager is responsible for managing various department programs providing patient support and linkage to care for a variety of priority populations. Programs include but are not limited to HIV/AIDS, Viral Hepatitis (HBV/HCV), Pre-Exposure Prophylaxis (PrEP) / Post-Exposure Prophylaxis (PEP), Cancer Prevention, Reproductive Life Planning, General Community Health and Health Promotion. Individual reports to the Department Director and is responsible for the following: Implementation and oversight of assigned programs, track measurable outcomes, Monitor program sustainability, and Report to partner agencies. It is imperative that this role be able to develop and implement strategies to increase screenings, linkage to care and utilization of care and treatment for patients served. In addition to managerial duties, the Program Manager will continue to engage in direct patient care, working as part of a multidisciplinary team while assisting patients with navigating the health care system. Provides individualized case management and/or care coordination, including but not limited to patient navigation, education, resources, referrals and advocacy for patients and their families.
Performance Requirements:
Ability to read, write and understand the English language Must have ability to work well within a diverse team. This would include but is not limited to communicating with community partners, managing data dissemination and coordinating project-related strategies with the team in order to lead program in achieving assigned goals. Must exhibit excellent interpersonal and communication skills, both oral and written (i.e. customer service, telephone skills, and e-mail communication), which are required for interacting with program team, program participants, organization staff and healthcare team Must follow established guidelines for assessing compliance with protocol and operational procedures. Communicate effectively with a wide range of patients, operations staff, providers and other departments in a friendly, professional manner incorporating a team-based approach to healthcare – working with the physical and emotional development of all age groups Knowledge of medical office protocols/procedures Ability to plan, prioritize and complete delegated task with attention to detail and efficient time management Ability to work well under pressure and with various time constraints is essential Ability to learn and retain information regarding patient care procedures Proficiency in computer operations, including but not limited to: MS Word- Excel-Outlook-PowerPoint Ability to work successfully with indirect supervision and autonomy Ability to work with culturally diverse people Typical work hours are from 8:00am to 5:00pm. Attendance at evening and/or weekend meetings and events may be required as necessary. Must have valid driver’s license and personal transportation to travel between office/county locations. Neat, well-groomed and professional appearance
Education: Bachelor’s degree in Social Work, Public Health or a discipline related to the field; All degrees must be received from appropriately accredited institutions. Master’s degree preferred in aforementioned fields.
Experience Required: Minimum 3-year experience working within clinical setting (public/community health) and/or with priority communities/patient populations. Supervisory experience preferred.

K ey Responsibilities:
Demonstrate and apply knowledge of medical conditions, including but not limited to medical terminology, transmission, risk reduction, disease referral resources and accessible treatment programs. Update/maintain accurate and secure patient medical records. Perform medical case management and/or care coordination duties to maintain knowledge of direct practice: Intake/assessments on new and established patients. Maintain up-to-date progress notes completed in a timely manner, according to department policy. Develop comprehensive plans of care (working with the patients and providers) to include goal setting to achieve and maintain optimal patient health while advocating in the best interest of the patient & organization. Coordinate/facilitate referrals in order to link patients to care/resources. Perform community outreach to promote programs, while establishing partnerships. Includes but not limited to attending community events, coordinating community meetings, providing educational presentations, etc. Provide programmatic oversight and leadership to assigned team, including but not limited to responsibilities of assigned program(s), communication with assigned team, & identify/solve process or personnel challenges. Complete and submit required programmatic reports to funders in a timely manner, according to contracts, as well as monitor programmatic expenditures and requirements to ensure contractual compliance. Actively participate in QI activities, in order to evaluate program activities to improve quality of care and program efficiencies, while monitoring progress toward goals. Participates in staff/committee/agency meetings as required, as well as leads meetings/huddles as requested. Assist with developing programming and new initiatives based on team and patient needs (e.g. support groups, educational sessions, new partnerships, etc.). Regularly updates Supervisor with changes and/or challenges to program(s), while performing other clinical or administrative functions per the Dept. Director.
