The Behavioral Health Counselor at Westside provides counseling with an emphasis on prevention, working with individuals and groups to promote optimum behavioral, mental and emotional health. The Behavioral Health Counselor may help individuals deal with issues associated with addictions and substance abuse; family, parenting, and marital problems; stress management; self-esteem; and aging. WORKING AT WESTSIDE MEANS WORKING IN A PRIMARY CARE MEDICAL HOME. A Primary Care Medical Home is not a special building. It is a way to provide healthcare that puts the patient at the center of health care decision-making. As a community health center, Westside Family Healthcare provides care for everyone who walks through our doors. OUR MISSION: To improve the health of our communities by providing equal access to quality healthcare.. OUR BENEFITS : Our benefit package includes medical insurance, dental insurance, vision insurance, life and disability insurance, a 401(k) retirement plan with a match, and supplemental insurances. We offer a generous PTO package and flexibility to provide work/life balance. CORE RESPONSIBILITIES Maintain confidentiality of records relating to clients' treatment. Collect information about clients through interviews, observation, review of medical records and/or tests and prepare and maintain all required treatment records and reports, entering information into the electronic medical records system in accordance with policy. Evaluate clients' physical or mental condition and develop, implement and modify treatment plans. Counsel clients or patients, individually or in group sessions, to assist in overcoming dependencies, adjusting to life, or developing skills or strategies for dealing with their problems. Act as client advocate to coordinate required services or to resolve emergency problems in crisis situations. Collaborate with other staff members to perform clinical assessments or develop treatment plans. Counsel family members to assist them in understanding, dealing with, or supporting clients or patients. Monitor clients' use of medications. Learn about new developments in counseling by reading professional literature, attending courses and seminars, or establishing and maintaining contact with other social service agencies. Gather information about community mental health needs or resources that could be used in conjunction with therapy and refer patients, clients, or family members to community resources or to specialists as necessary. Coordinate or direct employee workshops, courses, or training about mental health issues. Assist the primary care and behavioral health teams in developing care management processes such as the use of screenings, disease management techniques, case management and patient education.
