Our Physical Therapy professionals play a vital role in ensuring the health, safety, and well-being of our residents by providing high-quality, individualized therapy services that support recovery, mobility, and independence.</p>\n<p> </p>\n<p><strong>Responsibilities</strong></p>\n<ul>\n <li><p>Provide comprehensive physical therapy evaluations and treatments for residents in a skilled nursing setting</p></li>\n <li><p>Develop and implement individualized treatment plans based on resident needs and physician recommendations</p></li>\n <li><p>Administer skilled physical therapy interventions to improve mobility, strength, balance, and overall function</p></li>\n <li><p>Monitor resident progress and adjust treatment plans as appropriate</p></li>\n <li><p>Collaborate with interdisciplinary team members to ensure coordinated and effective care</p></li>\n <li><p>Maintain accurate and timely clinical documentation in accordance with regulatory standards</p></li>\n <li><p>Ensure the highest degree of quality care and resident safety is maintained at all times</p></li>\n</ul>\n<p><strong>Qualifications</strong></p>\n<ul>\n <li><p>Current <strong>State Physical Therapy License</strong> required</p></li>\n <li><p>Strong clinical assessment and treatment planning skills</p></li>\n <li><p>Ability to work collaboratively in a multidisciplinary team environment</p></li>\n <li><p>Commitment to providing compassionate, resident-centered care</p></li>\n</ul>\n<p><strong>Why Join Elderwood at Cheektowaga?</strong></p>\n<ul>\n <li><p>Supportive team environment</p></li>\n <li><p>Opportunity to make a meaningful difference in residents’ lives</p></li>\n <li><p>Competitive compensation and benefits (based on employment status)</p></li>\n <li><p>Professional growth opportunities within the Elderwood network</p></li>\n</ul>\n<p>If you are passionate about helping individuals regain independence and improve their quality of life, we encourage you to apply and become part of the Elderwood therapy team.</p>\n<h2>Responsibilities</h2>\n<p><strong>Physical Therapist:</strong></p>\n<ul>\n <li><p>Initiates skilled physical therapy treatments in accordance with physician’s orders; maintains required contact with physician.</p></li>\n <li><p>Maintains written evaluations, progress notes and discharge summaries for individuals in physical therapy program.</p></li>\n <li><p>Confers with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.</p></li>\n <li><p>Provides input for meetings and attends during absences of the Director Physical Therapy.</p></li>\n <li><p>Maintains statistical records of individuals’ treatments.</p></li>\n <li><p>Assists with in-service training programs upon request.</p></li>\n <li><p>Records and updates individual’s plan of care in the Inter-disciplinary Kardex.</p></li>\n <li><p>Submits requisition for supplies needed in the treatment to Director Physical Therapy.</p></li>\n <li><p>Assumes all duties of the Director Physical Therapy during absences including supervision of Physical Therapy Aides and responsibility for all necessary documentation.</p></li>\n</ul>\n<h2>From</h2>\n<h2>Up to</h2>\n<h2>Qualifications</h2>\n<p><strong>Physical</strong> <strong>Therapist</strong>:</p>\n<ul>\n <li>Graduation from an approved school of physical therapy required.</li>\n <li>State license to practice physical therapy required.</li>\n <li>Two years’ experience in physical therapy preferred.</li>\n <li>Ability to direct others necessary.</li>\n <li>This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
