    "requisitionDescription": "<div><div><div><p><br></p><p id=\"isPasted\"><strong><u>ACCOUNTABLE FOR:</u></strong> &nbsp;</p><p>The &nbsp;Nurse Aide Trainee works along a side Certified Nursing Assistant (C.N.A.) who is responsible for maintaining quality of care for all residents.</p><p><strong><u>GENERAL PURPOSE AND ESSENTIAL FUNCTIONS</u></strong><u>:</u></p><p>&nbsp;<strong>Responsibilities and Duties:</strong></p><ol start=\"1\" type=\"1\"><li>Review Activity of Daily Living sheets at the beginning of each shift to ensure that the plan of care if followed for each resident</li><li>Document on the ADL sheets as appropriate; document any changes observed with the residents, who you reported the changes to, any resident complaints, any pertinent information that is relevant to maintaining quality of care.</li><li>Document appropriately on the Dietary Log, Bath Log, Snack Sheets, Weekly Blood Pressure Sheets, ADL sheets, etc&hellip;</li><li>Report bowel movements and intake and output to the charge nurse at the end of each shift</li><li>Assist with admissions and inventory residents&rsquo; personal belongings.</li><li>Answer call light signals as quickly as possible.</li><li>Assist residents to be clean, neat, and well-groomed daily.</li><li>Change bed linens on bath days and as needed.</li><li>Make beds daily</li><li>Oral care every shift and as needed.</li><li>Nail care on bath days and as needed.</li><li>Incontinence care at least every two hours and as needed.</li><li>Catheter care every shift and as needed</li><li>Assist with bathing per schedule and as needed</li><li>Assist with toileting at least every 2 hours and as needed</li><li>Skin care every shift and as needed.</li><li>Shave men and women on bath days and as needed</li><li>Assist with meals; pass trays; set-up meal trays; feed as needed; pick up trays; clean resident&rsquo;s clothing &amp; chairs after meals.</li><li>Document meal percentage intake</li><li>Report any coughing, watering eyes, or choking which may indicate a swallowing problem.</li><li>Offer snacks, ice, water,drinks morning, afternoon, night, and as indicated.</li><li>Ensure that drinking cups are clean; if not, obtain a new one.</li><li>Turn and reposition residents who are in bed, wheelchairs, geri-chairs, etc&hellip;at least every two hours and as needed</li><li>Clean equipment as assigned and as needed (chairs, walkers, side rails, cushions, mats, restraints, etc&hellip;</li><li>Assist with range of motion exercises, ambulation, walk to dine, toileting, sit to dine, toileting, transfers, etc&hellip;</li><li>Assist with activities daily</li><li>Obtain vital signs as instructed.</li></ol><p>&nbsp;<strong>&nbsp;</strong><strong>Related Functions:</strong></p><ol start=\"1\" type=\"1\"><li>Arrives for the scheduled shift no earlier than fifteen (15) minutes before the shift begins and clocks out no later than fifteen (15) minutes after the scheduled shift ends.</li><li>Required to work weekends on a rotating basis.</li><li>Responsible for one overtime shift a month if needed.</li><li>Demonstrates loyalty to the organization, honesty, and integrity at all times in the care and use of patient / resident and facility property.</li><li>Demonstrates positive support of any and all programs / projects implemented by the DON or Administration.</li><li>Maintains confidentiality of patient / resident information.</li><li>Displays a neat, clean, and professional appearance at all times.</li><li>Cooperates when needed if the facility faces an emergency or unusual circumstance.</li><li>Participates in Continuous Quality Improvement activities through monitoring and evaluation of patient care.</li><li>May be asked to participate on committees and community projects.</li><li>Participate in staff meetings, in-service programs, performance improvement, infection control activities as necessary.</li><li>Promote teamwork with physicians, patients, families, visitors, and co-workers.</li><li>Use proper body mechanics for transfers, lifting, etc&hellip;</li><li>All other duties as assigned.</li></ol><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</p><p><strong>REQUIREMENTS:</strong>&nbsp; &nbsp; &nbsp; &nbsp;</p><p><strong>Education/Training Experience:</strong>&nbsp; &nbsp;</p><p>High School Diploma or GED equivalent.&nbsp;</p><p>Freedom from illegal use of drugs and freedom from use and effects of drugs and alcohol in the workplace. At least one year of clinical experience preferred.</p><p><strong>Verbal and Written Communication Skills:</strong> Must be able to read, write and speak the English language fluently. Must be able to relate to individuals with various ethnic, cultural, and socioeconomic backgrounds.</p><p><strong>Interpersonal Skills:</strong> Relates cooperatively and constructively with all patients, guests, families, physicians, peers, and all healthcare members as well as anyone that JHS&rsquo; employees interact with while performing their daily duties.</p><p><strong>Essential Technical/Motor Skills:</strong>&nbsp; Full Ambulation and precise eye-hand coordination. &nbsp;&nbsp;</p><p><strong>Essential Mental Abilities:&nbsp;</strong>Knowledgeable and competent to perform essential functions. &nbsp;Must be able to deal with a variety of personalities with whom the employer comes into contact with, both pleasant and irate. &nbsp;</p><p><strong>Essential Sensory Requirements:</strong>&nbsp; Ability to visually and audibly perform all essential functions.</p><p><strong>Essential Physical Requirements:</strong>&nbsp; Moderate to high amount of walking, standing, sitting, reaching, pulling, pushing, stooping, and kneeling.
