Description Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity. Wellness Assistant A PACE Wellness Assistant supports older adults by promoting strength, mobility, and overall well-being through individualized and group exercise programming. They lead and assist with therapeutic activities designed to improve balance, endurance, and functional independence, while encouraging healthy lifestyle habits and active engagement. The Wellness Assistant monitors participants’ responses to exercise, communicates progress or concerns to the interdisciplinary team, and collaborates closely with therapy staff to support safe and effective wellness interventions. Their role is centered on helping participants maintain independence and enhance quality of life through movement and activity. Qualifications Required Qualifications High school diploma or GED Basic caregiving or healthcare experience Ability to assist with activities of daily living (dressing, mobility, transfers) CPR and First Aid certification (or ability to obtain) Physical ability to lift and safely transfer participants Ability to pass background checks and health screenings Preferred Qualifications Experience working with older adults Previous work in PACE, assisted living, home health, or similar settings Knowledge of elder care and dementia support Strong communication and teamwork skills What’s in it for you?
