We are looking for a compassionate and skilled <strong>Licensed Practical Nurse (LPN)</strong>/Licensed Vocational Nurse (LVN) to join our team and help embody this mission. <strong>As an LPN</strong> you will deliver direct patient care based on approved treatment plans while adhering to clinical protocols and guidelines.  <strong>Now Hiring for South King County Areas!</strong></p>\n<ul>\n <li>Collaborate and coordinate with the RN Case Manager and other healthcare team members to ensure effective patient care according to the treatment plan and patient care guidelines.</li>\n <li>Conduct patient visits as specified by the physician’s treatment plan, ensuring compliance with federal, state, and county regulations.</li>\n <li>Make skilled observations of patients' physical and mental conditions, documenting care provided accurately.</li>\n <li>Report any changes in patient condition to supervisors and the attending physician in a timely manner.</li>\n <li>Provide patient education to patients and caregivers, fostering understanding of care plans and health management.</li>\n <li>Demonstrate sensitivity to the needs, customs, and feelings of patients and their families.</li>\n <li>Participate in case conferences and in-services or special case orientations.</li>\n <li>Complete and submit all required documentation accurately and promptly.</li>\n</ul>\n<p><strong>Your benefits include:</strong></p>\n<ul>\n <li>Competitive annual bonus structure to reward commitment and performance.</li>\n <li>Generous vacation policy, including paid holidays and personal days.</li>\n <li>Comprehensive medical, dental, and vision insurance plans.</li>\n <li>Tuition reimbursement for continued education and professional growth.</li>\n <li>Participation in the Fidelity 401(K) plan.</li>\n</ul>\n<h2>Job Requirements</h2>\n<ul>\n <li>Graduate of an accredited practical nursing program and licensed to practice in the respective state.</li>\n <li>Current CPR or BLS certification with hands-on demonstration is required and must be maintained throughout employment.</li>\n <li>Minimum of one year of nursing experience; one year of home care or hospital nursing experience preferred.</li>\n <li>Excellent clinical skills and strong written and verbal communication abilities.</li>\n <li>Ability to work effectively within a multi-disciplinary team and build interpersonal relationships.</li>\n <li>Strong organizational and time management skills.</li>\n <li>Valid Driver’s license and compliant auto liability insurance coverage as per organizational policy.</li>\n <li>Experience with electronic health records (EHR) systems, such as HomeCare HomeBase, is a plus.</li>\n</ul>\n<p> </p>\n<p>At CommonSpirit Health at Home, we are proud to be an Equal Opportunity Employer, promoting diversity, equity, and inclusion in every aspect of our organization.
