font-size:14px\">Provides outreach for the purpose of engaging clients into PSH services and orients potential clients to the H20 program.</span></li>\n\t<li><span style=\"display:block; font-size:14px\">Provides transportation to clients as appropriate and as medically necessary, and determined by the CLP team, in personal vehicle or in Terros Health provided vehicle, when available.</span></li>\n\t<li><span style=\"display:block; font-size:14px\">Ensures ongoing collaboration, including the communication of appropriate clinical information with other involved parties as appropriate.
