This on-site role provides practical nursing support, medication coordination, and recovery-oriented care for adults experiencing mental health and housing challenges within a supportive housing environment.</p><p style=\"margin-left: 0in; line-height: 1.5;\">As an LPN, you will help maintain medication systems, coordinate with pharmacies and providers, support continuity of care, and work closely with interdisciplinary staff to promote wellness and stability.
