Please fill this our with your application: <a href=\"https://docs.google.com/forms/d/e/1FAIpQLSdXLNiyirSxgBkqc-1hlxG1QVboCBQzDW2m9gFepm5WnUsPTg/viewform?usp=header\" target=\"_blank\" rel=\"noopener noreferrer\">Respite Provider Questionnaire</a>.<br><br>Sample Supports is actively seeking energetic and professional Respite Provider in the Boulder County area to provide regularly scheduled support to a young man living with intellectual and developmental disabilities. Services are provided on a regularly scheduled basis to provide regular caregiver breaks. Respite may be provided in the individual’s home/private place of residence or the private residence of a respite provider. Respite providers receive competitive payments to ensure that they are compensated for the difficulty of care required in supporting the placement. We over-communicate to ensure your unique needs as a provider are included in all individual services plans while also ensuring the best fit for the individual in services.<br></p>\n<p>Essential Duties/Responsibilities:</p>\n<ul>\n<li>Providing personal assistance, medical attention, emotional support, or other personal care to adults with intellectual and developmental disabilities.</li>\n<li>Maintain Medical Records and administer medicine for the individual</li>\n<li>Assist patients with daily activities</li>\n<li>Meet the needs as defined by the client’s service plan.</li>\n<li>Community access services that explore community services available to all people, natural supports available to the participant, and develop methods to access additional services/supports/activities needed by the participant.</li>\n<li>Travel services may include providing, arranging, transporting, or accompanying the participant to services and supports identified in the Service Plan.</li>\n<li>Supervision services which ensure the health and welfare of the participant and/or utilizing technology for the same purpose.</li>\n</ul>\n<p>Qualifications:</p>\n<ul>\n<li>Must be at least 18 years of age.</li>\n<li>Must be able to lift at least 75 pounds.</li>\n<li>Must have to have dependable and reliable transportation with the ability to safely transport</li>\n<li>Must have a valid driver's license</li>\n<li>Must meet approved motor vehicle record standards for providing transportation for individuals in services </li>\n<li>Must be able to pass Child/Adult Protection Services check </li>\n<li>Due to the nature of the work performed at Sample Supports, and that this position works with an at risk population, we do require a criminal background check be completed.</li>\n<li>Preferred knowledge and understanding of developmental disabilities, mental health, and trauma</li>\n<li>Must be competent and willing to perform Non-Violent Crisis Intervention physical managements as trained</li>\n<li>Job responsibilities include reporting any suspected maltreatment, abuse, neglect, or exploitation of an at risk individual.
