font-size: 12pt\">Develops individualized, participant-driven housing stabilization plans.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Engages landlords, property managers, and housing providers to secure housing placements.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Advocates for participants in housing negotiations, lease processes, and tenant rights education.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Coordinates financial assistance for rent, move-in costs, utilities, and other eligible housing-related expenses.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Ensures proper documentation and compliance with HRSN financial management and billing protocols.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Supports participants in accessing rental arrears assistance, utility programs, and other stabilization resources.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Provides ongoing case management including goal-setting, life skills coaching, and resource navigation.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Coordinates with healthcare providers, social service agencies, and employment programs to provide wraparound support.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Assists participants in obtaining identification, enrolling in benefits, and accessing supportive services.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Maintains accurate and timely case notes and documentation in HMIS or other required systems.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Tracks housing outcomes and participant progress to meet program reporting requirements.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Ensures confidentiality and compliance with Medicaid, county, HUD, and agency regulations.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Adheres to agency policies and best practices in trauma-informed care, cultural responsiveness, and harm reduction.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Performs additional duties as assigned in support of program operations.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Strong organizational and communication skills.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Ability to maintain confidentiality and manage sensitive participant information.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Proficiency in Microsoft Office, Google Suite, and client databases (HMIS preferred).</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Valid driver’s license, proof of insurance, and reliable vehicle required.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-family: Arial, sans-serif;
