It’s also about how we support one another and how we show up for our community. </p>\n<p> </p>\n<p>Together, we keep getting better - advancing our mission to transform healthcare and serve as a leader of positive change.</p>\n<p> </p>\n<p> </p>\n<p>The<strong> Manager, Social Impact & Population Health</strong> will lead and manage the team of Community Health Workers (CHWs) and the Program Coordinator to deliver community based support to high-risk low-income patients to help them reach their health goals.</p>\n<p> </p>\n<p><strong>This is a grant funded position.</strong></p>\n<h2>Responsibilities</h2>\n<p>A day in the life of a <strong>Manager, Social Impact & Population Health</strong> at <strong>Hackensack Meridian Health</strong> includes:</p>\n<ul>\n <li>Recruit CHWs through engaging with local community organizations.</li>\n <li> Organize and implement CHW training.</li>\n <li> Lead a team of CHWs who will work with hundreds of patients each year, assisting patients with getting medical care, setting and achieving health goals, and helping them stay healthy. </li>\n <li>Support, coach and mentor CHWs to ensure they provide their best work for patients. </li>\n <li>Create and Analyze reports to ensure CHWs are achieving the program goals. </li>\n <li>Integrate CHWs with care teams working in clinics and hospitals.</li>\n <li> Facilitate team meetings and support groups for patients.</li>\n <li> Manages the operating budget for area of responsibility.</li>\n <li> Attend a paid training program.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education, Knowledge, Skills and Abilities Required: </strong></p>\n<ul>\n <li>Master's in social work, public health, or equivalent is required. </li>\n <li>Three years leadership experience with non-profit or community based organization. </li>\n <li>Superior knowledge of healthcare and social service systems including NJ public benefit enrollment. </li>\n <li>Excellent understanding of the healthcare and social service systems including processes for enrolling in public benefits. </li>\n <li>Ability to analyze reports and use data to drive performance results. </li>\n <li>Ability to exercise judgment in the application of professional services.</li>\n <li> Excellent oral and written communication skills. </li>\n <li>Ability to carry out written and oral instructions.</li>\n <li> Passion for improving the lives of low-income individuals and families.</li>\n</ul>\n<p><strong> Education, Knowledge, Skills and Abilities Preferred: </strong></p>\n<ul>\n <li>LSW/MSW preferred. </li>\n <li>Must be proficient in MS Word and computer data entry. </li>\n <li>Prior experience as an outreach worker is a plus.</li>\n <li> Knowledge of the resources and community-based organizations across New Jersey is a plus. </li>\n</ul>\n<p> </p>\n<p>If you feel that the above description speaks directly to your strengths and capabilities, then please apply today!   </p>\n<h2>Starting Minimum Rate</h2>Minimum rate of $113,609.60 Annually\n<h2>Job Posting Disclosure</h2>HMH is committed to pay equity and transparency for our team members.
