This role focuses on improving health literacy and overall quality of life through comprehensive annual visits.</p><p>The NP will complete in-home annual wellness assessments (which may last up to one hour) and will evaluate medical history, current medications, health status, and social determinants of health.&nbsp;</p><p>This part-time position requires a minimum commitment of 20 hours per week. Availability can include the following:</p><ul><li>Weekdays: Monday through Friday, 8:00 a.m. &ndash; 8:00 p.m.</li><li>Weekends: Saturday and Sunday, 8:00 a.m. &ndash; 5:00 p.m.</li></ul><p>This position is pay-per-visit and will vary from $75-$125, depending on the type of visit. Travel reimbursement is also included at the IRS-approved rate of .70/mile. &nbsp;</p><p><strong>&nbsp;</strong></p><p><strong>DUTIES &amp; RESPONSIBILITIES</strong></p><ul><li>Travel up to 90% throughout designated regions, which may be updated based on business needs.</li><li>Conduct in-home assessments on identified patients, following established guidelines.</li><li>Visit types may include:</li><li>In-home Medicare and Medicaid Assessments.&nbsp;</li><li>In-home Well-Child Visits (WCV), for children ranging from 6 months to 21 years of age.</li><li>In-home Child and Adolescent Well-Care Visits (WCV).</li><li>In-home Annual adult Wellness Visits (AWV).</li><li>In-home Health Risk Assessments (IHRA).&nbsp;</li><li>Telephonic Adult Annual Preventative (AAP) Assessments.&nbsp;</li><li>Telehealth assessments, as needed.&nbsp;</li><li>Understand the importance of preventative health evaluations focused on monitoring physical, developmental, behavioral, emotional health, and the assessment of social determinants of health (SDOH).</li><li>Assess psychosocial and clinical needs of patients and families.</li><li>Coordinate and communicate care plans with primary care and community providers.</li><li>Support appropriate utilization of healthcare resources.</li><li>Ensure timely and accurate documentation across the continuum of care.</li><li>Formulate a list of current and past medical conditions using clinical knowledge, judgment, and current findings.</li><li>Identify urgent and emergent situations for proper intervention.</li><li>Educate members on topics related to health conditions and the continuation of care.</li><li>Identify diagnoses to be used in care management and active medical management.</li><li>Provide care coordination to alleviate identified needs related to behavioral,&nbsp;physical, and social issues.</li><li>Focus on addressing issues that will decrease medical costs and increase the quality of life.&nbsp;</li><li>Develop positive relationships with clients, caregivers, providers, and other&nbsp;team members.</li><li>Managing physical health problems through the provision of education.</li><li>Communicate gaps in care to the member&rsquo;s primary care physician if needed.</li><li>Home safety evaluations.</li><li>Identify internal and external referral needs when available.</li><li>Compliant with all HIPAA regulations and maintain the security of protected health information (PHI).</li></ul><p><br></p><p><strong>SKILLS &amp; QUALIFICATIONS</strong><strong>&nbsp;</strong></p><ul><li>One (1) year of experience as an NP is preferred.&nbsp;</li><li>Experience in the care of elderly population is preferred.</li><li>Experience in the care of pediatric populations preferred.&nbsp;</li><li>Experience in home health, physician&rsquo;s office, or public health is preferred.</li><li>Computer skills required: (Microsoft Office Suite Software).</li><li>Adapts to changes in policies, procedures, new techniques, &amp;
