This position focuses on helping tenants identify and secure safe, affordable housing and provides ongoing support to ensure long-term housing stability.</p><p>The Case Manager will link clients with community resources, assist with budgeting, tenancy requirements, and communication with landlords, and empower clients to reach personal goals and greater self-sufficiency. This role emphasizes a strengths-based approach&mdash;focusing on each client&rsquo;s unique capabilities to promote confidence and independence.</p><p><strong>Key Responsibilities:</strong></p><ul type=\"disc\"><li>Assist clients throughout the housing process, from application to lease-up.</li><li>Ensure clients understand and comply with lease terms and Fair Housing laws.</li><li>Develop individualized service plans and track client progress.</li><li>Link clients to community resources, benefits, and support programs.</li><li>Provide crisis intervention and coordinate care as needed.</li><li>Maintain accurate and up-to-date case files and data systems.</li><li>Facilitate tenant meetings or groups to build community and reduce isolation.</li><li>Collaborate with landlords, agencies, and internal teams to support housing stability.</li></ul><p><strong>Qualifications:</strong></p><ul type=\"disc\"><li><strong>Education:</strong> Bachelor&rsquo;s degree in human services or social-work related field (or equivalent experience)</li></ul><ul><li><strong>Experience:</strong>&nbsp;<ul><li>Experience working with individuals experiencing homelessness, mental health challenges, and/or substance use disorders.</li></ul></li></ul><p><strong>Additional Requirements:</strong></p><ul type=\"disc\"><li>Valid driver&#39;s license and access to a personal vehicle for regional and statewide travel.</li></ul><p>&nbsp;</p><p> If you are interested in learning more about how you can join our talented group of diverse employees, please visit our website to view this job posting and apply.
