In everything you do, you embody Legacy’s mission of making life better for others. </p>\n<p>If this sounds like you, we invite you to apply for our <strong>Diabetes Educator</strong> role.</p>\n<p><strong>Legacy Salmon Creek Medical Center</strong> is Southwest Washington’s most modern hospital, offering the latest technology in a setting designed for comfort and care for the whole family. We feature innovations in joint replacement, robotic surgery, pelvic health for women, cancer care, intensive care for newborns, neurosurgery, medical care for children and more.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Provides patient and family instruction about the care of diabetes.</li>\n <li>Assists in the development of diabetes education materials and programs in collaboration with the health care team.</li>\n <li>May participate in outreach education in rural communities.</li>\n <li>Provides education on diabetes to members of the health care team.</li>\n <li>Communicates ongoing analysis of patient and family needs, verbally or in writing, to appropriate health care team members.</li>\n <li>Completes a minimum of 15 hours of continuing education per year in diabetes and education principles.</li>\n <li>Provides individual assessment, goal setting, planning, implementation of plan, and evaluation and follow-up.</li>\n <li>Utilizes evidence-based guidelines to guide practice.</li>\n <li>Helps to develop and implement evaluation of educational offerings.</li>\n <li>Uses continuous quality improvement and LEAN principles to improve service to best meet customer needs.  Measures outcomes.</li>\n <li>Participates in development of educational offerings.</li>\n <li>Assists in development of the program plan and participates in yearly program review.</li>\n <li>Informs patients of the existence and sources of appropriate diabetes supplies.</li>\n <li>Provides for dissemination of information regarding research developments and diabetes.</li>\n <li>Networks with other diabetes educators to stay current on modalities of education.</li>\n <li>Participates in research projects when appropriate.</li>\n <li>Participates in the provision of cost effective patient education through the full continuum (inpatient, outpatient, home care).</li>\n <li>Assists in the development of system wide standards of care for diabetes education.</li>\n <li>Maintains education hours efficiently and productively according to budget plan.</li>\n <li>Utilizes appropriate community resources to assist patients and families as needed.</li>\n <li>Participates in the planning and presentation of community and professional educational programs.</li>\n <li>Participates in the diabetes community through work in community programs.</li>\n <li>Participates in media relations and coverage for the center when the opportunity presents itself.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education: </strong></p>\n<ul>\n <li>Registered Dietitian <strong>or</strong> Registered Nurse (BSN preferred).</li>\n <li>Completion of 30 hours of continuing education in diabetes education principles in prior two years.</li>\n</ul>\n<p><strong>Experience: </strong></p>\n<ul>\n <li>Minimum two years of experience in diabetes education in acute care or ambulatory care setting.</li>\n</ul>\n<p><strong>Skills: </strong></p>\n<ul>\n <li>Basic familiarity with computers preferred.</li>\n</ul>\n<h2>Licensure</h2>\n<ul>\n <li>Applicable state licensure. </li>\n <li>Certified Diabetes Educator preferred; certification required within 2 years of employment.</li>\n</ul>\n<h2>Pay Range</h2>USD $46.37 - USD $66.31 /Hr.\n<h2>Our Commitment to Health and Equal Opportunity</h2>\n<p>Our Legacy is good for health for Our People, Our Patients, Our Communities, Our World.
