When you join Dartmouth Health Home Care, you’ll become part of a dedicated team delivering outstanding home health and hospice services that enrich the lives of the people we serve. </p>\n<p> </p>\n<p><strong>Spiritual Care Counselors</strong> at DHHC are integral members of a multi-disciplinary health care team that are responsible for the provision of spiritual care services to patients and families/caregivers of the Agency, either directly or through coordination of care with other spiritual counselors. </p>\n<p> </p>\n<p><strong>The Spiritual Care Counselor will:</strong></p>\n<ul>\n <li>Assure spiritual assessment of patients and families in the hospice program and appropriate services are rendered in a timely manner to meet regulatory guidelines.</li>\n <li>Provide direct spiritual care to patients and families/caregivers.</li>\n <li>Serve as liaison to community chaplains and spiritual counselors.</li>\n <li>Document direct services and ongoing communication with community resources.</li>\n <li>Provide funeral or memorial services for patients as requested.</li>\n <li>Provide bereavement support to families and caregivers for up to a year after the death of the patient.</li>\n <li>Perform hospice informationals for patients and families</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Graduate of Theological Studies from accredited university required.</li>\n <li>Experience with database software or electronic medical record software required.</li>\n <li>At least 1 unit of Clinical Pastoral Education required.</li>\n <li>Completion of Clinical Pastoral Education residency preferred.</li>\n <li>Two years of experience in health-related field preferred.</li>\n <li>Hospice experience preferred.</li>\n</ul>\n<h2>Required Licensure/Certifications</h2>\n<p>None</p>",
