    "description": "<h2>Overview</h2>\n<p>The Clinical Dietitian is responsible for the evaluation, planning and clinical assessment of nutritional status of the residents and participants of Loretto long-term care facilities or sites. Coordinates all phases of nutrition care including nutrition assessment, care planning, monitoring, education and counseling of residents and participants in a variety of settings. Performs duties in compliance with licensing, accrediting, and regulatory agencies. Contributing member of the interdisciplinary team in establishing and implementing the interdisciplinary care plan.</p>\n<h2>Responsibilities</h2>\n<p>PACE CNY Specific Job Duties:</p>\n<ul>\n <li>Advise Diet Technician/Home Health Aides regarding food service operations in the Day Center.</li>\n <li>Attend daily meetings with the team and initial care planning and reassessment meetings.</li>\n <li>Complete assessment semi-annually and with significant change in status as indicated</li>\n <li>Active member of tissue trauma team.</li>\n <li>Adheres to the mission and philosophy of the D&TC, PACE CNY, and the Loretto Corporation.</li>\n <li>Adheres to service excellence by developing and maintaining positive respectful relationships will all customers, internal and external, to include participants, families, team members, all staff throughout the organization and community partners; performs responsibilities according to the highest quality standards.</li>\n <li>Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and care verbalize their role during an event.</li>\n <li>Any other duties required to meet the goals and objectives of the program.</li>\n</ul>\n<p>Job expectations for all employees:</p>\n<ul>\n <li>Promotes and role models the mission, values, vision and strategic goals of Loretto in all interactions with staff, peers, residents, family members, vendors and visitors. Identifies and reports or corrects any possible safety or environmental issues.</li>\n <li>Understands all HIPAA and other compliance & regulatory requirements associated with their role, including completion of required internal or external training/certifications.</li>\n <li>Performs a variety of related duties as assigned.</li>\n</ul>\n<p>INDPRO</p>\n<h2>Qualifications</h2>\n<ul>\n <li>Two years of clinical experience in a health care setting desired. Long-term care or outpatient experience preferred.</li>\n <li>Completion of a dietetic internship.</li>\n <li>B.A. or B.S. in dietetics from an AND (Academy of Nutrition and Dietetics) accredited academic institution, required.</li>\n <li>Current registration or registry eligible.</li>\n <li>Able to effectively assess residents or participants’ nutritional needs and establish plans of nutritional care.</li>\n <li>Ability to work effectively in a collaborative environment.</li>\n <li>Ability to communicate effectively with participants, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.
