Description
MyAdvisor/VetAdvisor care coordination program provides expert care for those that serve others. From behavioral health to career development, MyAdvisor will triage individual needs to find the best resources for ongoing support. With over 12 years of proven success, MyAdvisor provides support to those who are responsible for the safety and welfare of others. Three Wire delivers practice, preventative, and integrated care coordination to private and public sector employers. This care coordination, in turn, supports clinical tele-behavioral health, holistic wellness, navigational advocacy and family stability, and thus helps individuals thrive in their career and family life
Shift time: Mon-Fri 10:45am-7:15pm EST
JOB SUMMARY:
Provide coaching, care coordination, referral and appointing services to clients enrolled in the VA Caregiver Legal and Financial program. Provide services via phone, text and video and a case management system for case notes, appointing, and other documentation.
Essential Job Functions:
Responsible for assessing needs of participating clients provide education and assistance for the following legal services: Simple Wills, Advanced Directive, Power of Attorney, Durable Power of Attorney, Guardianship, Contract Disputes, and Civil Disputes. Responsible for tracking, monitoring, and documenting all coaching sessions and related services within the case management system. Provide connections to appropriate resources and make necessary referrals for facilitating goal attainment. Complete ongoing training as required by customer and MyAdvisor

Minimum Education/Training Requirements :
Paralegal Certification with at least two years’ experience assisting with the services we provide (listed above).
