Additionally, our Community Health Initiatives focus on addressing health disparities, making care more accessible to underserved populations.</p>\n<p><br>At POD Health, we strive to make healthcare personal, proactive, and accessible for all.</p>\n<p>                <br>Schedule: Monday – Friday | 9:00 AM – 5:00 PM |  Home Base: Tampa Palms, 33647, 100% Onsite</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Position Summary:  </span></p>\n<p><br></p>\n<p>POD Health is seeking a Licensed Clinical Social Worker (LCSW) to join our clinical team full-time, on-site in Tampa Palms, FL. In this role, you’ll support patients - by assessing mental health needs, developing care plans, and collaborating with a multidisciplinary team to ensure well-rounded support for conditions such as addiction, depression, and anxiety.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Key Responsibilities: </span></p>\n<p><br></p>\n<p>-Conduct thorough biopsychosocial assessments and develop personalized care plans.<br>-Focus on treating addiction, depression, and anxiety, primarily in senior populations.<br>-Liaise closely with the medical team to define care goals and manage patient outcomes.<br>-Educate patients about available services and promote active participation in their care.<br>-Maintain timely and accurate documentation in the Electronic Health Record (EHR).<br>-Participate in weekly clinical supervision and interdisciplinary team meetings.<br>-Uphold NASW ethical and professional standards in all areas of practice.<br>-Build therapeutic rapport with patients from diverse backgrounds while maintaining clinical boundaries.<br>-Responsible for outreach to Pod Health clients for enrollment in the Behavioral Health Integration (BHI) program<br>-Conduct outreach and follow-up calls with clients experiencing crisis situation<br>-Follow up on referrals received from the medical team to coordinate behavioral health support and services<br>-Schedule and conduct therapy sessions for clients through Zocdoc.</p>\n<p><br></p>\n<p><br><span style=\"font-weight: bold\">Required Qualifications:</span></p>\n<p><br></p>\n<p>-Master’s degree in Social Work from an accredited institution.<br>-Current LCSW license in Florida required.<br>-Experience supporting patients with addiction, depression, and anxiety.<br>-Strong interpersonal, communication, and documentation skills.<br>-Ability to work independently and collaboratively within a team-based model.<br>-Commitment to HIPAA compliance and maintaining confidentiality.<br>-Passion for serving aging and diverse communities.</p>\n<p>-Must be bilingual in Spanish.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Preferred Qualifications:</span></p>\n<p><br></p>\n<p>-Familiarity with telehealth platforms or virtual care settings.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Benefits:</span></p>\n<p><br></p>\n<p>-Health, dental, and vision insurance<br>-401(k) with company match<br>-Paid Vacation, Company Holidays, Sick Leave, and Floating Holidays</p>\n<p>-Professional development opportunities<br>-Mission-driven work in an innovative healthcare space</p>",
