medical conditions so that services are appropriate to changing needs.</span></div>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<div><span style=\"font-family:'Times New Roman',Times,serif\">Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services.</span></div>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<div><span style=\"font-family:'Times New Roman',Times,serif\">Provide counseling and orientation regarding tenant&rsquo;s rights and responsibilities to maintain and retain housing.</span></div>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<div><span style=\"font-family:'Times New Roman',Times,serif\">Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and provide transportation when necessary.</span></div>\r\n\t</li>\r\n\t<li dir=\"ltr\">\r\n\t<div><span style=\"font-family:'Times New Roman',Times,serif\">In collaboration with vocational counselors and entitlement specialists provide advocacy &amp;
