Description
Critical Tasks
Documentation Management Comprehensive Record Keeping : Letters of Intent (LOIs), 45-day reviews, amendments, closures, and transfers. Timely Filings : All relevant documents are filed with the appropriate commissioners promptly. Includes tracking deadlines and setting reminders to avoid any lapses in compliance. Care Coordination Appointment Follow Up : Follow up on 30-day appointments with Behavioral Health Medical Providers (BHMP) and Case Managers (CM). Collaborative Care Planning : Create comprehensive care plans for COT members, addressing their unique needs while facilitating a multi-disciplinary approach to treatment. Outreach and Support Engagement with COT Members : Conduct outreach efforts to connect with COT members, ensuring they are aware of and can access available resources and support services. Training and Development : Ongoing training and support for Case Managers (CMs), Clinical Coordinators (CCs), and GMHSU Clinical Managers to enhance their confidence and skills in managing COE and COT cases. This may include workshops, one-on-one coaching sessions, and developing training materials. Data Management Daily Updates : Collect, collate, and update tracking metrics essential to documentation for COT members by use of a comprehensive spreadsheet. C ollaboration with IT and Quality Management : Collaborate with Quality Management (QM) and IT teams to ensure that all documents are accurately uploaded and updated in NextGen and the designated P drive. Develops protocols for document management and ensures that all staff are trained in these processes. Legal Support Document Review and Notarization : Facilitate the review and notarization of legal documents, including DCS subpoenas for members. Petition Support : Support CCs in preparing petitions, including emergent petitions and Psychiatric Advance Directives (PAD). Training and Meetings: Ongoing Professional Development : Participate in COT trainings and other relevant coordination meetings in order to maximize effective contributions to the JFCS teams. Regular Meetings with SMI CCs, and Clinical Managers : Meet with CCs once a month, or as needed, to review case progress, discuss challenges, and brainstorm solutions. Collaborate with CD to consistently implement policies and procedures across the Clinical Coordinator environment. Assist teams in daily duties if a CC is out on PTO. Track and follow up with transfers/referrals from Mercy Care and other Health Plans. Supervision and Collaboration: Weekly Supervision Sessions: Weekly supervision with the SMI Clinical Director to discuss case progress, share insights, and strategize on improving care delivery for COT members. Cross-Departmental Collaboration : Actively collaborate with other stakeholders, including lawyers, public advocates, jails, county offices, public defenders, hospitals, guardians, and court coordinators.

Effectively protects the organization from potential legal issues. Facilitates continuity of care for COT members, ensuring that they receive necessary services and interventions without delay. Ensures regular communication and case review meetings. Utilizes various communication platforms and outreach strategies to engage members effectively. Maintains a keen understanding of legal standards and a commitment to safeguarding the rights of individuals involved. Stay current with best practices and regulatory changes within the behavioral health field.
Metrics
All records are accurate, organized, and easily accessible for audits and reviews. Consistent updating of Data management tool(s) to meet the goal of “real-time” data management. All filings are thorough and compliant with legal requirements.
Qualifications
Bachelor's Degree from an accredited college or university with a major in a mental health related field such as social work, counseling or psychology. A combination of pertinent experience and education may be considered. 3+ years’ experience as a direct service provider in a Behavioral Health setting. Supervision experience preferred and experience working with the Adult system of care preferred and/or working with teams of case management staff. Ability to exercise discretion and independent judgment with respect to matters of significance. Must be able to perform the essential functions of the job including prolonged sitting and in certain positions driving, lifting 20lbs, walking, standing and executing dexterity with a keyboard or smart pad. Ability to maintain DPS fingerprint clearance.
