Plans organize and conducts physical therapy treatment based on the medical referral and their evaluation.</p><p><strong>Patient Population</strong>: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations.</p><p><strong>Essential Functions:</strong></p><ol><li>Promote /exemplify Company mission, vision and values at all times.</li><li>Perform clinical assessments; identifies patient needs and appropriateness of client.</li><li>Document, prepare/revise and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis.</li><li>Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA).</li><li>Plan and provide appropriate treatment for neuro-muscular neurological and orthopedic disorders based on medical referral and therapy assessment.</li><li>Assist/instruct client/family in transfer techniques appropriate to client abilities. Instruct on use of equipment.</li><li>Provides effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case Conference/Team Meetings.</li><li>Monitors assigned cases to ensure compliance with the requirements of third-party payers.</li><li>Completes appropriate documentation in a timely manner to assure compliance with company policy.</li><li>Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency.</li><li>Meet mandatory continuing education requirements of the Agency/licensing board.</li></ol><p>This description is a general statement of required essential functions performed on a regular and continuous basis.
