    "description": "<p><strong>JOB SUMMARY: </strong></p>\n<p>We are seeking a compassionate and dedicated Home Health Aide to provide essential support to individuals in their homes or assisted living facilities. The ideal candidate will have experience in senior care and a genuine desire to work with patients during their end-of-life changes. </p>\n<p> </p>\n<p><strong>RESPONSIBILITIES</strong></p>\n<ul>\n\t<li>Perform personal care (bathing, dressing, toileting, feeding, etc) according to the Hospice Aide Plan of Care.</li>\n\t<li>Document care provided according to agency policy.</li>\n\t<li>Report any deviations from the Plan of Care to the Case Manager (or designee if not available).</li>\n\t<li>Use Durable Medical Equipment safely.</li>\n\t<li>Take vital signs when on Hospice Aide Plan of Care.</li>\n\t<li>Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.</li>\n\t<li>Provide proper care and observation of patient’s skin to prevent breakdown of tissue over bony prominence. Reports any changes/areas of concern to Case Manager.</li>\n\t<li>Assist with ambulation, transfers, range of motion if documented on the Hospice Aide Plan of Care.</li>\n\t<li>Provide respite for patient’s family/caregiver when on-site, as appropriate.</li>\n</ul>\n\n<p> </p>\n\n<p><strong>JOB TYPES: Weekends 7am-3pm, 8am-4pm, 830am-430pm </strong></p>\n\n<p><strong>Pay: $20 an Hour</strong></p>",
