POSITION SUMMARY
The Continuum of Care (CoC) Resident Case Manager provides comprehensive, client-centered case management services to LGBTQIA2S++ older adults and adults with disabilities experiencing housing instability, medical concerns, and complex psychosocial challenges in the Openhouse and Mercy Housing 55 and 95 Laguna Street properties. This position operates under contract with the City and County of San Francisco Homelessness and Supported Housing department and must comply with all applicable federal, state, local, and contractual requirements governing publicly funded case management programs. The CoC Case Manager performs assessments, care planning, coordination, advocacy, documentation, reporting, and monitoring functions to ensure compliance with Openhouse, Mercy Housing, and CoC standards and to support client safety, dignity, and housing stability.
ESSENTIAL FUNCTIONS
The following duties are considered essential job functions under the California Fair Employment and Housing Act (FEHA) and the Americans with Disabilities Act (ADA):
1. Client Assessment & Care Planning
Conduct intake, eligibility determination, and enrollment in accordance with CoC guidelines Perform comprehensive functional, psychosocial, and housing stability assessments Develop individualized, strengths-based service plans Conduct monthly client contacts (minimum) Complete annual reassessments and recertification of various public program benefits 2. Service Coordination & Advocacy
Coordinate housing, medical, behavioral health, and public benefits services (including Medi-Cal, IHSS, Medicare, and related systems) Advocate for clients experiencing housing, health, or service-related crises Collaborate with Mercy Housing property management and services staff, and community-based providers Maintain professional working relationships and boundaries with interdisciplinary service partners 3. Documentation, Data Entry & Contract Compliance
Maintain timely, accurate, and complete case documentation Enter and update required information in ONE System and Neon, and other mandated systems Maintain documentation consistent with CoC, HIPAA, and Openhouse confidentiality standards Participate in audits, monitoring reviews, and quality assurance processes Coordinate transportation to medical, and benefit-related appointments as required for care coordination 4. Mandated Reporting Responsibilities
This position is designated as a Mandated Reporter under California Welfare & Institutions Code §15630 (Dependent Adult and Elder Abuse Reporting Act). The CoC Case Manager is legally required to:
Immediately report suspected elder or dependent adult abuse to Adult Protective Services (APS) or appropriate authorities Complete required written follow-up reports within statutory timelines Participate in mandated reporter training as required by state law Maintain documentation of compliance with reporting obligations Failure to fulfill mandated reporting responsibilities may result in disciplinary action and potential legal consequences. 5. Team Participation & Professional Standards
Participate in weekly case conferences and supervision Maintain confidentiality in compliance with HIPAA and other privacy regulations Provide services in a culturally responsive, LGBTQ-affirming, and trauma-informed manner COC CASE MANAGER CONTRACTOR COMPLIANCE
As a HSH-funded position, the CoC Case Manager must:
Adhere to all CoC program standards and contract requirements Maintain documentation sufficient for City & County monitoring and audit Participate in required CoC trainings Meet productivity and reporting benchmarks as defined by contract Comply with City, State, and Federal nondiscrimination and confidentiality regulations Employment in this role may be contingent upon continued funding under the CoC contract.
