Patients in the SCI/D program receive five hours of therapy a day, five days a week, for an average of 6-12 weeks. Once enrolled in the SCI/D program, patients are evaluated and given a personalized treatment plan which can include one-on-one therapy sessions, group-based interventions and classes, education and specialized services to best address each patients’ individual goals and needs. The SCI/D team and program focuses on community reintegration and managing ongoing medical needs all while working towards a goal of a smooth transition into the community and next level of care.&nbsp;</p>\n<p>SCI/D is located in the Brooks Healthcare Plaza. The program has its own designated gym but with access to a wide range of equipment and technology in the Healthcare Plaza outpatient center. SCI/D also works closely with the Driving Program, School Reentry, Adaptive Sports program, NRC, Cyberdyne and Research.</p>\n<p><b>Responsibilities</b>:</p>\n<p>The Nurse Case Manager in the SCI/D Day Treatment Program is a member of the interdisciplinary&nbsp;team who provides timely and cost-effective coordination of rehabilitation services to best meet the individual's needs. &nbsp;The Nurse Case Manager promotes a positive outcome, serving as a link between patient, team, payer, and community. &nbsp;The plan-of-care facilitates communication among&nbsp;and between all parties, promotes patient-family decision-making, and maximizes service delivery transitions to return to the community&nbsp;</p>\n<p><b>Qualifications:&nbsp;&nbsp;</b></p>\n<ul>\n <li>RN or LPN License&nbsp;in the state of Florida&nbsp;</li>\n <li>2 years experience&nbsp;as a Nurse Case Manager in Acute Care or Inpatient Rehabilitation.&nbsp;</li>\n <li>Experience working with patients and families that have acquired spinal cord injuries&nbsp;is desired but not required</li>\n <li>Must be flexible to meet the needs of the patients and families</li>\n <li>Must have the ability to work with patients, families, staff, vendors, payors, or any stakeholder to ensure safe discharge&nbsp;and optimal outcomes for all of our patients.</li>\n <li>Must be versed in utilization management processes and able to balance the needs of patient advocacy &nbsp;with the &nbsp;needs of&nbsp; resource stewardship.</li>\n <li>Must be flexible, able to positively &nbsp;adapt to and quickly respond to daily, unexpected and varied environmental demands.</li>\n</ul>\n<p><b>Compensation</b>: Experience and education may be considered along with internal equity when job offers are extended<b>.</b></p>\n<p><b>Location:</b> 3901 University Blvd South, Jacksonville, FL 32216</p>\n<p><b>Hours:</b> Monday - &nbsp;Friday, 40 hours per week</p>\n<p><i>Thriving in a culture that you can be proud of, you will also receive many employee benefits such as the following:&nbsp;</i></p>\n<ul>\n <li><i>Competitive Pay&nbsp;</i></li>\n <li><i>Comprehensive Benefits package&nbsp;</i></li>\n <li><i>Vacation/Paid Time Off</i></li>\n <li><i>Retirement Plan</i></li>\n <li><i>Employee Discounts</i></li>\n <li><i>Clinical Education and Professional Development Programs</i></li>\n</ul>",
