The Opportunity Are you a Nurse Practitioner who believes that healthcare should meet people where they are – without judgement, without barriers, and with compassion at the centre of every interaction? The Gilbert Centre is looking for an experienced NP to join our HIV and HCV Clinical Services team. In this role, you will provide direct clinical care to people living with or at risk of HIV and hepatitis C – many of whom face significant barriers to accessing traditional healthcare. You will be part of a multidisciplinary team that practises harm reduction, centres client autonomy, and is guided by a deep commitment to health equity. This is not a supervisory role. It is a hands-on clinical position where you will work alongside an RPN, an RN, a Clinical Support Worker, Linkage to Care, Care Coordinators, and the Health Program Manager to deliver care grounded in justice, informed by lived experience, and responsive to the real needs of our communities. About Us The Gilbert Centre for Social and Support Services is a registered, not-for-profit charitable organization incorporated in 1995 in response to the HIV epidemic in Simcoe County and Muskoka Region. Our mandate extends to supporting people at risk of, living with, and affected by HIV and other sexually transmitted and blood-borne infections (STBBIs), as well as supporting and celebrating the lives of the 2S-LGBTQ+ communities in our regions. We take a broad, social-determinants-of-health approach to our work, grounded in social justice, health equity, and human rights. We seek to engage individuals, families, organizations, institutions, and communities as partners and collaborators. What You’ll Do Clinical Care Provide direct clinical care to individuals living with or at risk of HIV and/or HCV, including conducting in-depth and focused assessments, diagnosis, treatment initiation, health education/counselling and ongoing management. Review patient referrals and assess eligibility for treatment; consult with patients found ineligible and provide referrals to other health care services. Will feel comfortable providing consultations to nursing and other members of the patient care team and delegating tasks appropriately, on-site and remotely (if needed). Involve patients in all aspects of their treatment and care, including providing instructions and support for treatment administration, managing side effects, and developing follow-up plans. Maintain an understanding of available financial assistance and compensation programmes; assess patients' drug insurance coverage and assist with application forms as needed. Document each contact with patients or health care team members, including the reason for contact and actions taken, including charting client assessments, procedures, and follow-up per the College of Nurses (CNO) standard Stay informed about current best practices and regulatory changes in HIV/AIDS and hepatitis C care. Collaboration and Integration Collaborate with the Health Program Manager and clinical team to ensure integrated and comprehensive care for clients, including participating in huddles, case coordination, case conferences and consults Participates in peer chart audit reviews to ensure CNO standards are met. Contribute to the development and refinement of clinical policies and procedures specific to HIV/AIDS and hepatitis C care, optimizing patient care and ensuring compliance with relevant regulatory requirements and standards of practice. Ensure accurate documentation and record-keeping within the Centre’s electronic health record system (Accuro EMR) in accordance with legal and ethical standards, including adherence to privacy and confidentiality requirements under PHIPA and CNO standards Support community engagement and partnership activities to enhance access to clinical services for priority populations for HCV and HIV care, including outreach and pop-up clinics, education and presentations and other deliverables required by the funder. Contribute to the Gilbert Centre’s sense of community by interacting with participants and volunteers, and by participating in community programming as available. What You Bring Required Current College of Nurses of Ontario registration in the Extended Cla ss with a valid certificate of registration from the College of Nurses of Ontario, in good standing. Minimum of five (5) years of clinical experience, preferably in HIV and/or HCV services. Strong understanding of HIV/AIDS and hepatitis C prevention, treatment, and care, as well as harm reduction principles and practices, or willingness to learn Experience and comfort working with diverse, marginalized, and underserved populations, including people who use drugs, sex workers, people experiencing homelessness, newcomers, and 2S-LGBTQ+ communities. Proficiency in electronic health record (EHR) systems. Maintenance of additional professional liability insurance appropriate to the scope of services. A valid Ontario driver’s licence and preferably the use of an insured motor vehicle for work-related travel within the Simcoe Muskoka region. Preferred Community nursing experience. Experience with chronic illness. Knowledge of mental health and addictions. Experience working within a unionized environment. Proficiency in French (written and verbal).
