HHHRC bridges gaps and helps individuals access essential resources such as housing, health care, entitlements, and treatment. We foster health, wellness, and systemic change in Hawai&rsquo;i and the Pacific through outreach, care services, advocacy, training, prevention, education, and capacity building.</p><p style='margin:0in;font-size:15px;font-family:\"Calibri\",sans-serif;text-align:justify;'><br></p><p style='margin:0in;font-size:15px;font-family:\"Calibri\",sans-serif;text-align:justify;'><br></p><p id=\"isPasted\"><strong>Primary Purpose:</strong>&nbsp;</p><p>A Behavioral Health Case Manager is the key resource facilitator for people who face challenges with co-occurring social determinants of health, mental health, and substance use disorder. This entry-level case management position is responsible for providing effective case management to ensure clients are able to set, monitor, and achieve goals they have identified as they chart a path to stability and autonomy.</p><p>&nbsp;</p><p><strong>Reporting Relationship:</strong></p><p>Reports to: &nbsp; &nbsp; &nbsp; &nbsp; Behavioral Health Program Director</p><p>Supervises: &nbsp; &nbsp; &nbsp; &nbsp;N/A</p><p>&nbsp;</p><p><strong>Essential Duties &amp; Responsibilities:&nbsp;</strong><strong>&nbsp;</strong></p><ul><li>Engage referred clients, build trusting relationships, and provide individually tailored&nbsp;case management&nbsp;services.</li><li>Collaborate with care team to develop and implement an individualized health &amp; wellness plan based on client-identified goals.<ul><li>Update this plan periodically.</li><li>Reference in progress notes to reflect client change and to help the client meet their objectives.</li></ul></li><li>Assist&nbsp;with&nbsp;program&nbsp;evaluation&nbsp;by&nbsp;facilitating&nbsp;timely&nbsp;completion&nbsp;of&nbsp;required&nbsp;documentation, assessments and client encounters.</li><li>Advocate&nbsp;for&nbsp;clients&nbsp;to&nbsp;gain&nbsp;access&nbsp;to&nbsp;a&nbsp;wide&nbsp;variety&nbsp;of&nbsp;community&nbsp;resources.</li><li>Develop&nbsp;and&nbsp;maintain&nbsp;client&nbsp;charts&nbsp;and&nbsp;case&nbsp;notes&nbsp;for&nbsp;assigned&nbsp;caseload&nbsp;according&nbsp;to&nbsp;program, contract,&nbsp;and&nbsp;state&nbsp;requirements&nbsp;in&nbsp;digital&nbsp;and&nbsp;paper&nbsp;filing&nbsp;systems.</li><li>Perform&nbsp;other&nbsp;duties&nbsp;as&nbsp;needed&nbsp;or&nbsp;assigned.&nbsp;Duties&nbsp;may&nbsp;be&nbsp;subject&nbsp;to&nbsp;change.</li></ul><p>&nbsp;</p><p><strong>General Responsibilities:&nbsp;</strong>&nbsp;</p><ul><li>Contribute to a safe, creative, enthusiastic, and cooperative working environment for all.</li><li>Work as a team with other staff and support team members.</li><li>Maintain appropriate professional and ethical standards while serving as a representative of HHHRC.</li><li>Observe and comply with all rules and regulations &ndash; such as HHHRC Code of Conduct, Drug-Free Workplace, Confidentiality &ndash; and other administrative policies of HHHRC.</li><li>Abide by all policies and procedures of the HHHRC Health &amp; Safety Program and the Quality Management Program.&nbsp;</li><li>Comply with HHHRC confidentiality policy, HIPAA requirements, cultural competencies, and rights to persons served, as well as CARF standards.</li><li>Demonstrate steadfast understanding of, and commitment to, the Mission, Vision, and Values of HHHRC.&nbsp;</li></ul><p><strong>&nbsp;</strong></p><p><strong>Work Hours:&nbsp;</strong>&nbsp;</p><ul><li>Monday - Friday, 8:30am to 5:00pm,&nbsp;with some evening and weekend attendance required for agency events.</li></ul><p><strong>&nbsp;</strong></p><p><strong>Evaluation Metrics:</strong><strong>&nbsp;</strong></p><ul><li>Maintain an active caseload of a minimum of 30 clients, with variable on acuity.</li><li>Update health &amp;
