This position is responsible for pharmaceutical direct patient care and medication management for ambulatory patients, development of interdisciplinary pharmaceutical education activities for health care professionals, participation in performance improvement activities, and development of sustained partnerships with providers across the Foundation Health Partners system.
Pay & Benefits:
Compensation: $67.05 to $93.29 hourly wage based on experience and education
Retention Bonus: $40,000 Retention Bonus for eligible new hires
Relocation Assistance: up to $7,500 Relocation Assistance

Responsibilities
Conduct a chart review and patient medication history upon referral from physicians and other medical personnel.
Given a diagnosis, synthesize a medication treatment plan, which may include drug therapy and lifestyle modifications.
Under a collaborative practice agreement, initiate, adjust, monitor, and recommend changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, physical assessment, review of drug profiles and the patient medical records
Identify, resolve, and prevent potential and actual medication-related problems including untreated diagnosis, improper drug selection, therapeutic duplication, sub-therapeutic dosage, overdose, adverse drug reactions, drug interactions, and therapy without indication.
Coordinate and interpret laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing.
Practice shared decision-making with patient and referring provider with regard to optimization of medication therapy and barriers to compliance.
Provide patient education related to medication use, storage, safety, side effects, and disposal.
Develop and deliver pharmacy-related education to primary care providers across the Foundation Health Partners system.
Review and revise policies, procedures, and practice agreements related to ambulatory care pharmacy services on an annual or semi-annual basis.
Participate in performance improvement projects assessing outcomes related to ambulatory care pharmacy services.
Promote the optimal use of antimicrobial agents in the ambulatory care setting through participation in FHP antimicrobial stewardship and infection prevention activities.
Actively participate in relevant FHP committees as a clinical pharmacy representative.
Qualifications
Requires pharmacy licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy.
Pharmacy licensure in good standing. Immunization certification and/or licensure (if state requires) in state of practice required within 6 months of hire.
Must have at least one of the following; A PGY1 with two residency rotations in a relevant specialty or Board Certification issued by the BPS in a relevant specialty or subspecialty (BCPS, BCACP) applicable to ambulatory care, or 2-5 years relevant clinical pharmacy experience and Board Certification within 2 years of hire.
Ability to meet requirements for credentialing and privileging requirements as an advanced practical professional with the hospital medical staff.
Must possess good organizational, problem solving and computer skills. Must possess good written and verbal communication skills for interfacing with all levels of staff, physicians, patients and other contacts. Must be able to use reference tools to evaluate patient care and answer questions posed by customers.
Must have knowledge of computer software, particularly medication management information system software, and Microsoft applications.
Preferred Qualifications
2-5 years clinical pharmacy experience in an ambulatory care setting under a Practitioner-Pharmacist Collaborative practice agreement
Completion of a PGY-2 in Ambulatory Care Pharmacy
Board Certification issued by the BPS in Ambulatory Care Pharmacy (BCAPC)
