San Antonio Gardens in Pomona, California, </strong>approximately 30 miles outside of Los Angeles. This position is 40<strong>% direct patient care </strong>and 6<strong>0% wellness engagement and community outreach.</strong> Our client is a prestigious 5-star CCRC spanning 38 acres with nearly 500 residents, in a 40-year partnership with Sodexo. Considered a showcase account, Mt. St. Antonio Gardens is home to a close-knit, EDEN-trained team and an affluent community of residents. If you're excited about enriching the lives of seniors through nutrition and compassionate care, this is a fantastic opportunity!</p>\n<p> </p>\n<p><strong>Position is open to new and experienced Dietitian's!</strong></p>\n<p> </p>\n<p>Start with 3 Weeks' Vacation and 3 Personal Days!</p>\n<p>Reimbursement for Academy dues, state licensure fees and CDR renewal</p>\n<p>Money toward continuing education events</p>\n<p>Up to $5,000 tuition reimbursement annually</p>\n<h2>What You'll Do</h2>\n<ul>\n <li>Provide clinical nutrition care primarily within a 40-bed SNF, with occasional support across LTC, assisted living, and independent living</li>\n <li>Provide wellness engagement opportunities to the residents and employees as assigned;</li>\n <li>Consistently meet DOH/state/federal and company standards;</li>\n <li>Serve as an active member of the interdisciplinary team, working closely with nursing & medical staff and attending care conferences as assigned;</li>\n <li>Perform quality improvement initiatives such as test trays, food service audits, and meal rounds;</li>\n <li>Consult with independent living residents, and collaborate with onsite CDM</li>\n</ul>\n<h2>What We Offer</h2>\n<p>Compensation is fair and equitable, partially determined by a candidate's education level or years of relevant experience. If you need assistance with the application process, please complete this form.</p>\n<h2>Qualifications & Requirements</h2>\n<p>Minimum Education Requirement:</p>\n<p>Master's degree with completion of required coursework and supervised practice as dictated by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) </p>\n<p>For those eligible for the registration exam prior to 1/1/24:</p>\n<p>Bachelor's degree with completion of required coursework and supervised practice as dictated by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) </p>\n<p> </p>\n<p>Credentials Requirement:</p>\n<p>Meets eligibility requirements for Registered Dietitian Nutritionist (RD/RDN) by the Commission on Dietetic Registration (CDR) and will become registered within 120 days of hire.  In some states and/or facilities the RD/RDN credentials is required at the time of hireCertification/licensure as required by state(s) of practice.
