Position Title: Urgent Care Registered Nurse
Department: Nursing
Supervisor: Director of Nursing
Status/Hours Per Week: Non-exempt / 40 hours
Pay Range: The expected base pay for the position is $36-$65.02 per hour, based upon experience, and may be increased based on other factors, such as language, certifications, etc.
Primary Function :
The Urgent Care (UC) Registered Nurse functions under the supervision of the Director of Nursing and overall guidance and consultation with Primary Care Providers. The UC RN conducts initial clinical assessment of patients presenting to Urgent Care, determines the relative urgency of care for each patient and provides quality nursing care as part of the Urgent Care team.
Duties & Responsibilities :
Triage patients on arrival to UC, assess acuity, and prioritize care appropriately.
Perform clinical duties including taking vital signs, administering medications, wound care, immunizations, point-of-care testing, EKGs, and assisting with procedures.
Document patient assessments, interventions, and outcomes accurately and timely in the electronic health record (Epic).
Support providers with exams, treatments, and procedures as needed.
Provide patient education and discharge instructions, reinforcing treatment plans and follow-up recommendations.
Recognize and respond to urgent or emergent patient conditions, escalating to the provider or emergency services as necessary.
Function as part of the UC team, working collaboratively with medical assistants, providers, and other UCC staff.
Facilitate access to Urgent Care for hospital discharges that are being followed up on at the health center.
Provide follow-up to patients seen in walk-in as appropriate (e.g. picking up prescriptions, progress of recovery/medication adherence etc.).
Conduct nursing visits.
Check DAILY or MONTHLY per review schedule and record or report irregularities for the UC fridge, Emergency Code Cart and Medication Closet.
Participate as part of the team responding to UCC emergency (code blue) alerts.
Participate in supporting providers in reporting all diseases cited in the Massachusetts Department of Public Health list of Reportable Diseases to the State and Local Board of Health when applicable.
Provide patient education as needed.
Participate in department and health center meetings.
Maintain current knowledge of developments in nursing field and be responsible for self-development through continuing education opportunities.
Perform other related duties as required.
Qualifications
Minimum Basic Knowledge :
Licensed Registered Nurse in state of Massachusetts, (B.S.N. preferred).
Knowledge of clinical assessments and triage protocols in a community health center setting.
Experience & Qualifications :
Two years of nursing experience, preferably in urgent care or ambulatory care.
Familiarity with electronic medical records [Epic] or willingness to learn.
Current BLS certification, ACLS/PALS.
Strong clinical assessment and triage skills.
Excellent communication and interpersonal skills.
Ability to manage patients with a diverse set of needs.
Strong verbal and communication skills.
Ability to work collaboratively as part of an interdisciplinary team.
Critical attention to detail and ability to work under pressure and establish priorities.
Experience working in an FQHC, community health center, or similar setting desirable.
Bilingual language skills in Spanish or Cape Verdean Creole preferred.
Knowledge of multilingual patient population, sensitivity to socio-economic, trauma, and other factors impacting communities in need.
Familiarity with UCC neighborhoods and agencies.
Independent Action :
• Effective and efficient triage of patients presenting to UC, and prioritization of care.
• Ability to work efficiently under pressure and adapt to changing priorities.
• Effective and efficient delivery of services to a multicultural group of patients, with sensitivity to socio-economic, trauma, and other factors impacting communities in need.
• Effective communication and sharing of information with patients, providers and external partners.
• Collaborative approach to working with interdisciplinary team of internal and external providers.
• Complete and accurate documentation of care provided to UC patients.
Physical Requirements :
• Capacity to transport patients or things from place to place while bearing the weight of 0-30 pounds.
• Capacity to raise objects from lower to higher position, from higher to lower position and horizontally of 0-30 pounds.
• Capacity to stand and walk and for prolonged periods of time during shift and use stairs frequently and travel between floors in the building.
