    "description": "<h2>Salary</h2>Starting at $67,000 / yr\n<h2>Overview</h2>\n<p>Elderwood Independent Practice Association (IPA) is seeking a Social Worker Care Manager who has their MSW to join our incredible growing team of professionals.</p>\n<p> </p>\n<p><em>Why should you join Elderwood IPA?</em></p>\n<ul>\n <li>Monday-Friday Schedule w/ no weekends or overnights</li>\n <li>Hybrid working options</li>\n <li>Medical, Dental & Vision upon 1st of month following 60 days of hire</li>\n <li>Up to 140 hours (19 days) of PTO time PLUS six (6) holidays within the 1st year</li>\n <li>401(k) retirement plan with vested employer match up to 4%</li>\n <li>Substantial Referral program</li>\n</ul>\n<h2>Join Our Team</h2>Are you looking to take the next step in your career?\n<h2>Responsibilities</h2>\n<p>In the role of an Social Work Care Manager, you will partner with the Registered Nurse and are a critical resource for our members as you are responsible for obtaining needed medical, social, educational, psychosocial, financial, and other services, irrespective of whether the needed services are covered Elderwood IPA. </p>\n<ul>\n <li>Direct telephonic and in-person engagement with individuals with chronic medical illness or mental health issues and their families.</li>\n <li>Utilize assessment tools to determine the needs of our member population </li>\n <li>Link members to resources in the community outside of the scope of services MLTC provides to support social determinants of health. </li>\n <li>Interacts continuously with members, family, physician(s), and other resources to determine appropriate behavioral action needed to address/support medical needs.  </li>\n <li>Reviews benefits options, research community resources, coordinates services, trains/creates behavioral routines and enables members to be active participants in their own healthcare.</li>\n <li>Documents in the EMR each encounter or routine contact with members, providers or caregivers and reports changes in conditions to the RN Care Manager, which may require interventions.</li>\n <li>Participate in Interdisciplinary Team meetings, Disease Management, Utilization Management, and Quality Improvement activities and prepare/submit activity reports.</li>\n <li>Provide coaching and education to members and their caregivers on health care related issues </li>\n <li>Provide discharge planning and continuous care management across care settings and coordinate the transition planning </li>\n <li>Participate in shared on-call, after-hours coverage along with other Elderwood staff.</li>\n</ul>\n<h2>From</h2>\n<h2>Up to</h2>\n<h2>Qualifications</h2>\n<ul>\n <li>Master’s degree in social work (Required)</li>\n <li>Minimum of one (1) year Social Work experience with at risk populations in home care, case management, discharge planning or managed care. </li>\n <li>Minimum of one (1) year experience working with a frail or elderly population and/or members with chronic illness or behavioral health issues.</li>\n <li>Previous Managed Care experience, within a MLTC or working with special needs populations preferred.</li>\n</ul>\n<ul>\n <li>Must have a valid NYS Driver’s license, a safe driving record and dependable transportation</li>\n <li>Must be in good standing with the Medicare and Medicaid programs. </li>\n <li>Must be free of communicable disease.</li>\n <li>Bilingual skills (preferred)</li>\n</ul>\n<h2>EOE Statement</h2>WE ARE AN EQUAL OPPORTUNITY EMPLOYER.
