MHCP will coordinate with existing outpatient providers and natural supports in an effort to understand the current crisis, identify solutions, and access resources and services as needed. MHCP services are intended to support and maintain individuals in their current living situation and community environment, while reducing the use of emergency departments, hospital boarding, and
