    "requisitionDescription": "<div><div><h1 style=\"margin-left:9.15pt;\" data-pasted=\"true\"><span style=\"font-size: 14px;\"><u>POSITION OVERVIEW</u></span></h1><p style=\"margin-left:27.15pt;\">Member of the BCORE team to provide clinically oriented outreach and engagement, clinical casecoordination and therapeutic counseling for individuals who use or abuse opiates and other substances, diagnosed with a mental health disorder or dually diagnosed. Conduct community-based, assessments, consultation, treatment planning, implementation and follow-up on direct services and rehabilitation to assist clients and their families in coping with health-related social problems, addiction, mental illness, and trauma.</p><h1 style=\"margin-left:27.25pt;\"><span style=\"font-size: 14px;\"><u>REPORTING STRUCTURE</u></span></h1><p style=\"margin-left:30.25pt;\">There is no staff reporting to this position.</p><h1 style=\"margin-left:27.3pt;\"><span style=\"font-size: 14px;\"><u>SPECIFIC RESPONSIBILITIES</u></span></h1><p style=\"margin-left:45.35pt;\">Work effectively and collaboratively with other specialists as part of an interdisciplinary team. Utilize evidence-based practices for street outreach, clinical case management, addiction disorders, and co-occurring disorders when performing engagement, individual sessions and group sessions.</p><p style=\"margin-left:45.4pt;\">Conduct canvassing and outreach visits to various street locations, agencies, shelters, and hospitals to engage clients who may be eligible for the BCORE program.</p><p style=\"margin-left:45.25pt;\">Earn and maintain clients&#39; trust through consistency, integrity, honesty, managing</p><p style=\"margin-left:45.6pt;\">expectations, unconditional positive regard, and persistent follow-up; impart this approach throughout the team through role modeling, coaching, and close consultation.</p><p style=\"margin-left:45.3pt;\">Conduct screening interviews and assessments using personal interviews, referral information,</p><p style=\"margin-left:45.4pt;\">medical history, collateral contact and additional methods as needed.</p><p style=\"margin-left:45.4pt;\">Recommend treatment plans that address medical, psychiatric, and psychosocial needs/problems and utilize clinically appropriate and evidence-based interventions to address specific goals. Modify the plan as necessary to reflect the needs of the client.</p><p style=\"margin-left:45.35pt;\">Provide case management services for a group of assigned people which includes coordinating and monitoring the Individual Treatment Plan for the team.</p><p style=\"margin-left:45.4pt;\">Provide ongoing assessment of clients&#39; addictions and mental health and their response to treatment.</p><p>&nbsp;</p><div class=\"fr-page-break\" contenteditable=\"false\" style=\"break-after: always;\" data-title=\"PageBreak\"><div class=\"fr-page-break-line\"><span class=\"fr-page-break-label\">Page Break</span></div></div><p>&nbsp;</p><p style=\"margin-left:45.3pt;\">Provide symptom education and illness management as prescribed by evidenced based practices or established guidelines.</p><p style=\"margin-left:45.3pt;\">Provide individual therapy/treatment and ongoing counseling to individuals, groups, or families as reflected in the Individual Treatment Plan.</p><p style=\"margin-left:45.35pt;\">Ensure clients&#39; safety and priority social needs are met, including safe, affordable housing, adequate food, and clothing.</p><p style=\"margin-left:45.15pt;\">Assist clients to carry out daily living skills including personal hygiene, nutritional needs, shopping, and food preparation as well as house cleaning and laundry. Teach transportation skills, financial and budgeting skills.</p><p style=\"margin-left:45.3pt;\">Provide assistance with housing and benefits applications, including but not limited Coordinated Access, SSI/SSDI, and SNAP and participate in professional development to maintain benefits expertise.</p><p style=\"margin-left:45.3pt;\">Build strong relationships with clients, families, co-workers, and the surrounding community. Provide education to families and other identified natural supports.</p><p style=\"margin-left:45.65pt;\">Identifies gaps in service, barriers to treatment, and the acquisition of community resources. Advocates for the reduction of such barriers and for the development of services needed.</p><p style=\"margin-left:45.3pt;\">Provide crisis intervention and rapid support services to prevent hospitalization.</p><p style=\"margin-left:45.35pt;\">As needed, visit clients and potential clients who are in Emergency Departments or admitted to a hospital and assist with discharge planning and care coordination.</p><p style=\"margin-left:45.3pt;\">Provide on-call crisis intervention when assigned covering nighttime and weekend hours. Participate in BCORE-related meetings, client care conferences, training programs, etc. Keep up-to-date, quality services documentation following PEP, HMIS, and other approved policies and procedures.</p><p style=\"margin-left:45.35pt;\">Ensure timely and appropriate follow-up to referrals and excellent communication and coordination with referral partners.</p><p style=\"margin-left:45.15pt;\">Follow training in Housing First, Harm Reduction, Motivational Interviewing, Trauma/Resilience, Peer Support, Person-Centered Care Coordination/Case Management, Street Medicine, and other evidence-based practices.</p><p style=\"margin-left:45.3pt;\">Provide on-call and/or rotating shift duty as needed on nights and weekends and during emergencies.</p><p style=\"margin-left:45.3pt;\">Perform urinalysis and other lab work as clinically indicated.</p><h1 style=\"margin-left:27.15pt;\"><span style=\"font-size: 14px;\"><u>PRINCIPAL ACCOUNTABILITIES</u></span></h1><p style=\"margin-left:27.05pt;\">Clinically oriented Community Canvassing and Outreach Clinically effective treatment plans</p><p style=\"margin-left:27.05pt;\">Completion of pertinent client documentation Quality of engagement and clinical services Liaison with outside agencies,</p><p style=\"margin-left:27.05pt;\">Client well-being (clinical and physical)</p><h1 style=\"margin-left:27.2pt;\"><span style=\"font-size: 14px;\"><u>EDUCATION, WORK EXPERIENCE AND JOB REQUIREMENTS</u></span></h1><p style=\"margin-left:27.25pt;\"><u>Education:</u> Master&#39;s degree in social work, or professional counseling</p><p style=\"margin-left:27.2pt;\"><u>Skill Requirement</u>: One to three months OJT required for efficient job performance.&nbsp;</p><p style=\"margin-left:27.2pt;\"><u>Job Requirements</u>:</p><ul style=\"margin-left: 53.45px;\"><li>Be a licensed Mental Health Professional in Maryland; minimum of LMSW or LGPC is required.
