Description

Position Title: Case Manager Position Status: Full-Time Supervisor: Case Manager Supervisor Work location: Community Salary Code: N Pre-Employment Requirements: Drug test Criminal Background check – over three years since prior conviction TB skin test: Must have a test or provide proof of one within the past year. Must have a valid driver's license, reliable transportation, and current automobile insurance. Must have a minimum of a Bachelor's degree in social work, psychology or related field. Prefer 2 years’ experience in chemical dependency. Must complete TCM Core Training and SUD training within 6 months of hire date Position Summary: Access, coordinate and monitor services to assist the client in becoming stable in the community and reaching the highest level of potential with alcoholism or substance abuse. Skill Sets: Demonstrate good oral and written communications. Maintain a supportive and encouraging working relationship with co-workers and clients. Participate in the initiation, coordination, and implementation of the referral plan. Follow through, monitor, and evaluate activities related to the recovery of the client. Must have basic computer skills. Physical and Environmental Demands: Visual acuity and manual dexterity are needed for the required paperwork. Frequent talking and hearing are required when communicating with co-workers, families, and other service providers. Physical demands are light with walking, standing, and occasional stooping. Job Responsibilities: Able to provide customer service to personnel, persons served, families, and stakeholders. Well-maintained health, safety, and welfare of personnel, persons served, and stakeholders. Ensure the agency is operating under CARF accreditation standards. Ensure that state and federal licensure and regulations are being adhered to. Individual assessment of Basic needs such as shelter, food, clothing, entitlements, etc. Specialized needs such as substance abuse treatment services, rehabilitation, social opportunities, vocational and employment capabilities, and psychiatric services. Develop and facilitate life skills learning to help clients prepare for post-program living. Determine clients’ needs for childcare and transportation in order to access services. Send letters to DCBS for clients when they begin working, so they do not lose insurance coverage. Provide ongoing contact with community agencies to develop additional resources and coordinate services. Develop and monitor a treatment plan for each client in the assigned caseload. Advocate for the client. Assist clients with the problems they face in early recovery, especially legal issues, lack of housing, and unemployment. Complete necessary initial documentation with clients including Assurance of Services, Needs Assessment, and Service/Treatment Plan. Complete 5 monthly contacts and document with client- 3 of which must be face to face. Complete Monthly Summary for each client. Maintain a caseload of up to 25 clients. Willing to have on-call responsibilities. Oversee the application process to obtain food stamps for clients. Coordinate with Operations Team on all insurance matters including informing them when a client begins employment. Must maintain TCM credentials by completing 6 CEUs annually. Other duties as assigned General Standards: Exemplify Core Values Follow the Ethical Code of Conduct Follow the Chain of Command Maintain Confidentiality of all client information Maintain all requirements for job duties to include but not limited to TB skin test and any continuing education requirements necessary.
