This role supports medical staff in delivering preventive care, health maintenance, and treatment for acute and chronic conditions.</p>\n\n<p><strong>Position Summary</strong></p>\n\n<p>The LPN&rsquo;s primary focus includes coordination of care across the continuum and chronic disease population management, while also providing patient education under the supervision of a medical provider.</p>\n\n<p>This position also participates in quality improvement initiatives, clinic operations, and may oversee students and allied health professionals during their educational experiences.</p>\n\n<p><strong>Essential Functions</strong></p>\n\n<ul>\n\t<li>\n\t<p>Maintain efficient patient flow, including taking vital signs and rooming patients</p>\n\t</li>\n\t<li>\n\t<p>Perform nursing procedures, therapies, tests, and services within scope and as ordered</p>\n\t</li>\n\t<li>\n\t<p>Promote a safety-conscious work environment</p>\n\t</li>\n\t<li>\n\t<p>Assess, plan, implement, and evaluate age-appropriate patient care</p>\n\t</li>\n\t<li>\n\t<p>Communicate effectively with diverse patient populations</p>\n\t</li>\n\t<li>\n\t<p>Prioritize and manage patient care (in-person visits, calls, and written requests)</p>\n\t</li>\n\t<li>\n\t<p>Support provider schedules and ensure efficient clinic operations</p>\n\t</li>\n\t<li>\n\t<p>Coordinate patient care across services, providers, and care settings</p>\n\t</li>\n\t<li>\n\t<p>Assist in chronic care management (e.g., depression, diabetes, CHF, chronic pain)</p>\n\t</li>\n\t<li>\n\t<p>Administer medications, perform waived testing, EKGs, and treatments</p>\n\t</li>\n\t<li>\n\t<p>Prepare patients for exams and assist with procedures</p>\n\t</li>\n\t<li>\n\t<p>Educate patients and families on health topics under provider supervision</p>\n\t</li>\n\t<li>\n\t<p>Monitor and report patient conditions and responses to treatment</p>\n\t</li>\n\t<li>\n\t<p>Obtain and document patient histories</p>\n\t</li>\n\t<li>\n\t<p>Maintain accurate patient records and perform data entry</p>\n\t</li>\n\t<li>\n\t<p>Handle patient calls and provide treatment guidance as appropriate</p>\n\t</li>\n\t<li>\n\t<p>Practice within scope of license and clinic protocols</p>\n\t</li>\n\t<li>\n\t<p>Adhere to all JST and JFHC policies, including safety and HIPAA standards</p>\n\t</li>\n\t<li>\n\t<p>Perform additional duties as assigned</p>\n\t</li>\n</ul>\n\n<p><strong>Requirements</strong></p>\n\n<ul>\n\t<li>\n\t<p>Graduate of an accredited school of nursing</p>\n\t</li>\n\t<li>\n\t<p>Current Washington State LPN license</p>\n\t</li>\n\t<li>\n\t<p>Current First Aid/CPR certification</p>\n\t</li>\n\t<li>\n\t<p>Knowledge of nursing theory, regulations, and patient care practices</p>\n\t</li>\n\t<li>\n\t<p>Understanding of safety, infection control, and environmental precautions</p>\n\t</li>\n\t<li>\n\t<p>Strong documentation and record-keeping skills</p>\n\t</li>\n\t<li>\n\t<p>Demonstrated clinical competence</p>\n\t</li>\n\t<li>\n\t<p>Ability to maintain high standards of nursing care</p>\n\t</li>\n\t<li>\n\t<p>Excellent interpersonal, verbal, and written communication skills</p>\n\t</li>\n\t<li>\n\t<p>Basic computer proficiency</p>\n\t</li>\n\t<li>\n\t<p>Valid driver&rsquo;s license</p>\n\t</li>\n\t<li>\n\t<p>Ability to pass a criminal background check</p>\n\t</li>\n</ul>\n\n<p><strong>Preferred Qualifications</strong></p>\n\n<ul>\n\t<li>\n\t<p>Prior clinic experience</p>\n\t</li>\n\t<li>\n\t<p>Experience with EPIC electronic health records</p>\n\t</li>\n\t<li>\n\t<p>Strong organizational and multitasking abilities</p>\n\t</li>\n\t<li>\n\t<p>Effective time management, problem-solving, and decision-making skills</p>\n\t</li>\n\t<li>\n\t<p>American Indian/Alaska Native preference applies</p>\n\t</li>\n</ul>\n",
