    "description": "<p><strong>Comunilife Inc</strong> is looking for an experienced <strong>Mental Health Counselor for its OTDA Program. </strong>The Mental Health Counselor provides mental health related services for individuals with chronic health issues. clients with mental health needs and their families. Responsible for the timely completion of mental health assessment and collaborating in the development and implementation of service plan(s) for each client admitted to the program. <strong>This position is eligible for Comunilife&#39;s generous benefits and paid time off package and it is salaried up to $60,000 a year.&nbsp;</strong></p>\r\n\r\n<p>Schedule:</p>\r\n\r\n<p>Mon.-Fri. 9am - 5pm</p>\r\n\r\n<p><strong>The program sites are located in the Bronx and Brooklyn boroughs. Travel within those locations will be required.</strong></p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><strong>Essential functions</strong></p>\r\n\r\n<p>Conduct mental health assessments and assist in the development and implementation of comprehensive service plans that identify mutually agreed upon long and short-term mental health related goals.</p>\r\n\r\n<p>Provide mental health services and referrals; assistance with adherence to medication regiments; activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training; prevention with positives; and other necessary services to clients to increase their capacity for independent living. Also, monitor and ensure that clients are linked to needed services.</p>\r\n\r\n<p>Provides, coordinates and collaborates with providers within and outside Comunilife.</p>\r\n\r\n<p>Provide supportive counseling to clients and coordinates and collaborates with mental health providers to help clients to comply with treatment and medication regime.</p>\r\n\r\n<p>Make home visits and maintains face to face contact as specified in the service plan(s)</p>\r\n\r\n<p>Responsible for developing and implementing weekly groups including the Wellness Self-Management model.</p>\r\n\r\n<p>Responsible for the timely completion of all required documentation including, comprehensive assessments, reassessments, progress notes of all contacts with clients and/or collaterals.</p>\r\n\r\n<p>Participate in case conferences, training, workshops and seminars as well as meetings with peers and supervisors as required.</p>\r\n\r\n<p>Assist in the planning and organizing program recreational activities.</p>\r\n\r\n<p>Participates in weekly supervision.</p>\r\n\r\n<p>&nbsp;</p>\r\n",
