General Summary/Responsibilities: Status: Full-time: *40 Hours per week (80 hours per pay period) - 0700-1530 Department: Pharmacy Facility: Miami Valley Hospital Under the supervision of the System Director, Pharmacy Clinical Services and according to standard procedures, provides clinical pharmaceutical care; coordinates pharmaceutical care with intensive care health care providers, practitioners, and educators to ensure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective medication therapy; optimize medication use through drug order review, therapeutic drug monitoring, participation in high-risk emergencies including resuscitation; supervises support personnel in all work areas assigned; precepts PGY1 and PGY2 pharmacy residents and pharmacy students; participates in clinical research protocols and drug therapy research activities as applicable. The clinical pharmacy specialist (tier 4) provides for staff development activities and the coordination of staff operational activities within medical intensive care and other assigned areas; serves as educational and informational resource to physicians, medical residents, advanced practice providers, pharmacy staff, nursing staff, patients, and other allied health professionals; assists with planning, organizing, and implementation of clinical pharmacy programs across Premier Health; identifies processes and tools to guide the pharmacy staff in the utilization of the most appropriate and cost-effective medication therapy including drug preparation and procurement, ensuring high quality, safety, and positive patient outcomes.

Minimum Level of Education Required: Bachelor%27s degree

Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) obtained within 90 days of employment start date

Essential Duties & Functions:
Optimize pharmaceutical care through oral and/or written communication with providers, nurses, and other healthcare professionals
Provide consultative services to providers, nurses, and other healthcare professionals to ensure optimal cost-effective pharmaceutical care to intensive care patients
Participate in multidisciplinary patient care rounds at Miami Valley Hospital
Participate in associated multi-disciplinary committees
Participate in code pink or code blue in area of practice
Flexibility in scheduling essential, with proper notification to meet departmental workloads
Familiar with all duties and procedures performed by professional and technical staff
Periodic weekend staffing required
Contributes to the development, review, or improvement of clinical or operational policies/guidelines/protocols/order sets
Creates in-services or presentations to pharmacy staff/other health professionals at organization
Assist in preventing adverse effects, reporting / investigating suspected or documented adverse drug reactions and/or medication errors
