POSITION DESCRIPTION JOB TITLE:              Food Service Worker/Job Coach (ASL Required) SALARY:                  DOE REPORTS TO:         Beary Cool Yogurt Supervisor _______________________________________________________________________ Hearts and Hands of Care is looking for a Food Service Worker/Job Coach to join our team. This Food Service Worker/Job Coach will work in a yogurt shop location, assisting the Supervisor and effectively communicating with American Sign Language (ASL) staff to achieve profitable sales in the Wasilla location. BASIC FUNCTION: This position involves assisting an individual experiencing developmental disabilities in the acquisition of work maturity skills necessary to obtain employment and independently maintain employment in the community. Job Coaches provide ongoing support to the consumer working at various employment sites within the community. This support is individual-based and centered around work-related goals that focus on helping the consumer obtain the maximum level of independence within their job. Job Coaches provide the support necessary to ensure success at future employment opportunities. Service will be provided at the consumer’s place of employment. ESSENTIAL DUTIES AND RESPONSIBILITIES:   Communicate effectively with the client using ASL. Consistently model appropriate work maturity skills and hold consumer to the same expectations. Always uphold standards of work contract, which may include finding a substitute if consumer leaves early or is absent. Promotes strengths-based perspective while empowering the individual to reach desired outcomes. Monitor consumer’s work performance and provide feedback and prompts when needed. Ensure safety is maintained in the work environment at all times. Know and adhere to established agency policies, procedures, and regulations. Attending agency and program orientation session. Demonstrating competency in completing all required forms, timecards, and reports. Consistently receive satisfactory performance evaluations. Provide service for the consumer regardless of location by creating and maintaining an atmosphere that is welcoming, understanding, communicative, and supportive of the consumer. Willing and able to substitute consumer services for other HAHOC Job Coaches, when needed. KNOWLEDGE, SKILLS AND ABILITIES :   Fluency in ASL to communicate with client Strong interpersonal skills and customer service Must have knowledge of the HIPAA laws of 1996 to maintain and safeguard confidential medical and personal records. Must have proven ability to provide descriptive notes addressing interventions used and individual response in database. Working knowledge of signs and symptoms of common illnesses and conditions Effective written and oral communication skills Ability to consistently be at work and on time. Ability to maintain a confidential working relationship. Excellent personal organizational skills Ability to follow oral and written instructions. Ability to communicate effectively and maintain a positive attitude in difficult situations. QUALIFICATIONS:   Must be at least 18 years of age; and have a High School diploma or equivalent. Possess and maintain a valid Alaska driver’s license and insured vehicle that meets minimum State of Alaska insurance requirements. Able to pass and provide a Background Check that meets State of Alaska requirements. Must be able to communicate and support the client who uses ASL. Able and knowledgeable to cope with emergency situations (i.e. accidents, CPR, seizures, etc.), in a calm and efficient manner. The description is a general statement of required major duties and responsibilities performed on a regular and continuous basis. It does not exclude other duties as assigned. Fluency in American Sign Language (ASL) to communicate with the client. WORK ENVIRONMENT:   Position may require extended hours including evenings and weekends. Ability to stand for prolonged periods of time. This Food Service Worker/Job Coach will work in a yogurt shop location, assisting the Supervisor and effectively communicating with American Sign Language (ASL) staff to achieve profitable sales in the Wasilla location.</b></span></span><br><br><span style=\"font-size:12pt;\"><span style=\"font-family:'Times New Roman', serif;\"><b>BASIC FUNCTION: This position involves assisting an individual experiencing developmental disabilities in the acquisition of work maturity skills necessary to obtain employment and independently maintain employment in the community. Job Coaches provide ongoing support to the consumer working at various employment sites within the community. This support is individual-based and centered around work-related goals that focus on helping the consumer obtain the maximum level of independence within their job. Job Coaches provide the support necessary to ensure success at future employment opportunities. Service will be provided at the consumer’s place of employment.</b></span></span><br><br><span style=\"font-size:12pt;\"><span style=\"font-family:'Times New Roman', serif;\"><b>ESSENTIAL DUTIES AND RESPONSIBILITIES:</b></span></span><br> <ul><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Communicate effectively with the client using ASL.</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Consistently model appropriate work maturity skills and hold consumer to the same expectations.</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Always uphold standards of work contract, which may include finding a substitute if consumer leaves early or is absent.</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Promotes strengths-based perspective while empowering the individual to reach desired outcomes.</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Monitor consumer’s work performance and provide feedback and prompts when needed. </b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Ensure safety is maintained in the work environment at all times.</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Know and adhere to established agency policies, procedures, and regulations.</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Attending agency and program orientation creating and maintaining an atmosphere that is welcoming, understanding, communicative, and supportive of the consumer.</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Willing and able to substitute consumer services for other HAHOC Job Coaches, when needed.</b></span></span></span></li></ul><br><br><span style=\"font-size:12pt;\"><span style=\"font-family:'Times New Roman', serif;\"><b>KNOWLEDGE, SKILLS AND ABILITIES</b><b>: </b></span></span><br> <ul><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Fluency in ASL to communicate with client</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Strong interpersonal skills and customer service</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Must have knowledge of the HIPAA laws of 1996 to maintain and safeguard confidential medical and personal records. </b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Must have proven ability to provide descriptive notes addressing interventions used and individual response in database. </b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Working knowledge of signs and symptoms of common illnesses and conditions</b></span></span></span></li><li><span style=\"font-size:12pt;\"><span><span style=\"font-family:'Times New Roman', serif;\"><b>Effective written and oral communication skills
